Art in Motion Dance & Gymnastic Academy
Acknowledgement of Risk & Liability Release Form
I understand that there are risks of physical injury, accidents, and/or death associated with,
arising out of, and inherent to the activity of dance and gymnastics. In recognition of this
acknowledged risk of injury and/or death, I knowingly and voluntarily waive all rights and/or
causes of action of any kind, including any and all claims of negligence arising as a result of
such activity from which liability could accrue to Art In Motion Dance & Gymnastics Academy,
its owners, employees, instructors, assistants, volunteers, guest artists, faculty members, students,
and/or all affiliated entities (hereinafter collectively referred to as “Art In Motion Dance &
Gymnastics Academy”.
I hereby agree to release Art In Motion Dance & Gymnastics Academy and hold Art In Motion
Dance & Gymnastics Academy harmless of all liability arising from any accident, personal
injury, death, or property loss or damage sustained by my child/myself/the minor child for whom
I am a legal guardian, while that person is participating in activities connected with Art In
Motion Dance & Gymnastics Academy. I hereby acknowledge that I knowingly and voluntarily
assume full responsibility for all risks of physical injury, death, or property loss or damage
arising out of participation in any dance or gymnastics classes, workshops, intensives, camps,
parades, rehearsals, or performances on behalf of the participant.
Furthermore, I understand and acknowledge that due to the physical nature of dance and
gymnastics, there may be physical contact between instructors, employees, assistants, staff, and
students during classes, workshops, intensives, rehearsals, or performances and especially during
dance and gymnastics instruction. I understand that for proper dance instruction, spotting
purposes in gymnastics, and for safety, physical contact is at times required and necessary.
I also give Art In Motion Dance & Gymnastics Academy permission to use photos and videos of
my child/myself/minor child for whom I am a legal guardian for promotional/advertisement
purposes, including social media, for Art In Motion Dance & Gymnastics Academy or an Art in
Motion Dance & Gymnastics Academy affiliated event.
If I am a minor, my parent or legal guardian has signed this document releasing Art In Motion
Dance & Gymnastics Academy from any and all such liability described above and has
acknowledged that I am knowingly and voluntarily assuming all risks of injury and/or death
inherent to the activities surrounding dance and gymnastics.
The participant has my permission to participate in Art In Motion Dance & Gymnastics Academy
events. I warrant the below information is complete and correct. I further release Art In Motion
Dance & Gymnastics Academy of all liabilities associated with my child’s attendance at Art In
Motion Dance & Gymnastics Academy.
