Photo & Video Waiver

Impetus Staff or contractors will at times take photos during class/rehearsals/performances to capture the excitement and keep as memories. I give permission for Impetus Dance Theatre to use video footage or photographs of me or my child/ward. This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs. I acknowledge that I am giving unrestricted permission for my child’s image to be used in print, video, and digital media. I agree that these images may be used by Impetus Dance for a variety of purposes and that these images may be used without further notification. I do understand that any identifying information including surname and location will not be used in conjunction with any video or digital images.
If I disagree with this and do not want my children's photo to be taken or shared under any circumstances, I agree to immediately inform Impetus Dance Staff via email and at the beginning of every calendar year. In this instance, I will also ensure that my child is aware that he/she is not to be photographed and will actively avoid being captured through notifying their teacher or the photographer when images are being taken.
