POLICIES AND PROCEDURES

TUITION:
Please be advised that you are signing a contract to pay the full amount of tuition. THERE ARE NO FULL OR PARTIAL REFUNDS ON TUITION. Tuition is based on the number of classes for the season (September 2024– May 2025) Tuition is the same no matter how many classes are in a month. Tuition is run on Auto pay every month on the 28th of the month before classes begin.

WITHDRAWAL:
Parents must give a 30- day written notice to withdraw their child from a class. Tuition is due until the written notice is received. THERE ARE NO FULL OR PARTIAL REFUNDS ON TUITION.

COLLECTIONS:
If you are more than 2 months behind in your tuition, and all previous attempts to collect have been unsuccessful (to include automatic withdraw), your account will be sent to United PAC’s law firm for collections and an additional $150 collection fee will be added your account. United Performing Arts Company reserves the right to remove a student from classes due to delinquent payments or non- payments. If a student is dismissed from the class for any reason or she chooses to quit the class, the full amount of the tuition will be accelerated and immediately due in full.
CLASS CHANGES
Students may not add or change a class without first notifying the front desk. A charge of $25 will be to the student’s account if they are found taking a class they are not enrolled in.

HOLIDAYS:
We will follow the AISD school calendar for holiday closings. Please remember there is no pro- rating for holidays.

ATTENDANCE:
When students are late to class, it is very disruptive to the instructor and the other students. United PAC reserves the right to have students who arrive more than 15 minutes late to class, sit out of the class. Repeated tardiness may result in the termination of lessons without a refund. No exceptions.
A minimum attendance level is required in order for the student to learn the choreography and participate in the performances. If a student misses more than 4 classes without written notice, the studio reserves the right to terminate lessons without a refund. Students missing more than 4 classes after the month of January may not be allowed to perform in the end of the year recital.

CONDUCT:
United Performing Arts Company expects outstanding behavior from all of its students. Each student’s behavior affects the morale of the class and the success of the recital. Examples of inappropriate behavior include, but are not limited to: name-calling, disrespecting the dance instructor or other team members (this includes gossiping about other team members), foul language, physical harm to anyone, and use of alcohol and/or drugs.
If a student’s behavior becomes inappropriate, the following procedures will be followed:

1st infraction: Student will be removed from class and placed in the lobby for the remainder of the class. A verbal warning will be given to the student. The student’s file will be noted.
2nd infraction: Student will be removed from class. A conference will be held with the parents to discuss the inappropriate behavior, and a plan will be implemented so that the behavior can be avoided in the future. Student’s file will be noted.
3rd infraction: Student will be withdrawn from the class. No refund will be given. No Exceptions!

PARENT RESPONSIBILITY:
It is the responsibility of the parent or adult student to be aware of all studio activities, such as observation days, recitals, extra classes, and dates the studio is open/closed. It is the parent’s responsibility to regularly check with the studio or the United PAC website to ensure they are informed. It is also the responsibility of the parent or adult student to inform the school of any address or telephone number change.
United PAC has an open-door policy and we welcome your concerns, suggestions and comments. However, we ask that each family approaches United PAC in a respectful manner and at an appropriate time.

SAFETY:
WE ARE NOT A DAY CARE FACILITY!!! We are not responsible for students before and after their class time, and after they have exited the studio. Please instruct your child to remain inside at all times. Students should be dropped off 5 min before class and promptly picked up after class. If a situation arises wherein a family is not complying with this policy, a conference will be held with the parents. If the situation is still not corrected after the conference, United PAC reserves the right to terminate lessons without a refund.

DRESS CODE:
All United students must be in dress code at all times. If a student is not in dress code, they will not be allowed to participate in class. Dress code for each class is available at the front desk and on the United PAC website.

MAKE UP CLASSES:
Students are welcome to do make-ups in any class that is age appropriate. Call the front desk in order to schedule. 817-504-7894

LOST AND FOUND:
These items will be placed in a bin behind the reception desk. These items will be donated monthly. United is not responsible for lost or stolen items at any time.

NEWSLETTERS/EVENTS/PERFORMANCES:
It is the parent’s responsibility to make sure they keep up with all performances. We will be putting everything on e-mail and the website this year to communicate all United PAC even

WAIVER:
I authorize United Performing Arts Company to use photos taken of my child while at dance or dance functions, for marketing and promotional material.
HOLD HARMLESS AGREEMENT:
In consideration for receiving permission to participate in the classes at United Performing Arts Company, I hereby RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE, United Performing Arts Company, its officers, agents, servants, or employees (hereinafter referred to as RELEASEES) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my child, or any of the property belonging to me or my child, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES, or otherwise, while participating in such activity, or while in, on or upon the premises where the activity is being conducted.
I am fully aware of the unusual risks involved and hazards connected with this activity, including but not limited to travel risks, broken bones, sprains, falls, etc. I hereby elect to HAVE MY CHILD voluntarily participate in said activity with full knowledge that said activity may be hazardous to my child and my property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OF LOSS, PROPERTY
DAMAGE OR PERSONAL INJURY, INCLUDING DEATH, that may be sustained by my child, or any loss or damage of property owned by me, as a result of my child being engaged in such activity, WHETHER CAUSED BY THE NEGLIGENCE OF RELEASEES OR OTHERWISE.
I further hereby AGREE TO INDEMNIFY AND HOLD HARMLESS the RELEASEES from any loss, liability, damage or costs, including court costs and attorney fees, that they may incur due to my participation in said activity, WHETHER CAUSED BY NEGLIGENCE OF RELEASEES or otherwise.
I understand that United Performing Arts Company does not maintain any insurance policy covering any circumstance arising from my participation in any event or any activity associated with or facilitating that participation. As such, I am aware that I should review my personal insurance portfolio. Further, if I have specific requests about which medical facility my child should be transported to in case of an emergency, I am to indicate that request below.
It is my express intent that this Waiver of Liability and Hold Harmless Agreement shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representative, if I am deceased, and shall be deemed as a RELEASE, WAIVER, DISCHARGE AND COVENANT NOT TO SUE the above- named RELEASEES. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be construed in accordance with the laws of the State of Texas. IN SIGNING THIS RELEASE, I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing Waiver of Liability and Hold Harmless Agreement, understand it and sign it voluntarily as my own free act and deed; acknowledge no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made; I am at least eighteen (18) years of age and fully competent; and I execute this Release for full, adequate and complete consideration fully intending to be bound by same. I am also releasing the permission of United Performing Arts Company to take photos of my dancer to post on social media for advertising purposes.


Your signature verifies that you understand and accept the policies and waiver of United Performing Arts Company LLC, and agree to abide by such terms.
