1. Premiere Studio of Dance & Teachers assume no responsibility for accident or injuries incurred in class or aggravation of pre-existing injuries or illness.
2. Premiere Studio of Dance may use photos any/or videos of my child in promotional material.
3. I hereby certify that my child is in good physical condition and able to participate fully in this program, and that all current medical conditions & required medications have been clearly outlined to the Studio.
4. I have read, and agree to abide by, the Studio policies outlined in the 2023-24 Fall Registration Package (available on premieredance.ca).
