I give my permission for my child to have photographic and/or video images taken while at The Dance Department Co. during instructional time and to be included in the Showcase Performance and DVDs.
I give my permission for my child to have photographic and/or video images taken while at The Dance Department Co. for Advertising purposes. I understand that I am giving consent to TDDC, its Directors, Staff, Local Newspaper Staff, Photography Staff, and other TDDC cooperating entities to use my child’s name, photograph, artwork, video or audio recordings for use in any media including social media for editorial, educational, promotional, or advertising purposes.
