Holds Harmless Agreement
Studio B Dance Center strongly advises that each participating student/client obtain a physical examination prior to class participation. Each participant is required to be in good health and have full health coverage. In reading this I have full knowledge that by nature, class participation can frequently include strenuous activities, exercise, and body contact along with associated risks. In recognition of the above said, guardians of/and participating clients hereby knowing and voluntarily waive any and all rights and all causes action of any kind whatsoever arising, directly or indirectly as the result of any such activity from which any liability may accrue against Studio B Dance Center, together with its directors, employees, agents or instructors. I also agree to waive any claims against any person or organization connected with Studio B Dance Center for any injuries I may sustain in connection with any program I am involved with or through Studio B Dance Center and likewise assume full responsibility and liability for all of my actions in connection with said classes, program, and activities at Studio B Dance Center or connected with the Studio B Dance Center.
Consent for Medical Treatment of a Minor
I understand that participation in activities through Studio B Dance Center could include actions or tasks which might be dangerous to the participating student. If the participant is a minor, I agree that the minor has my consent to participate in the event. I further provide my consent for Studio B Dance Center to seek emergency treatment for the minor if necessary. I agree to accept financial responsibility for the costs related to this emergency treatment.
Media Use Rights
I give permission for Studio B Dance Center to take photos of me or my child while participating in Studio B Dance Center activities for promotional purposes. Names of students will not be used or disclosed, without the permission of the individual if over 18 and the guardian if under 18r. I understand that for the safety of our dancers and their families, all photos and or videos of Studio B Dance Center classes, rehearsals and/or performances, including dancers in Studio B Dance Center costumes, will not be published or posted publicly, in printed or electronic format, without the express written permission of Studio B Dance Center and the Director.
Children's Privacy Policy
At Studio B Dance Center, children's safety is paramount. We will never publish personal information for students of any age. Information provided during class registration will be kept completely confidential. We will never publish a student's first and last name, with the exception of event programs, without receiving prior permission. An example of this case is a student receiving special recognition for excellence. There may be occasions where a student's first name may be used for editorial reasons or caption a photograph. We welcome family participation and some features on our website may allow access to videos for rehearsal purposes. Children under 13 should not access any interactive portion of our website without first obtaining parental permission. If a child under 13 years of age wants to participate on our website, we encourage the child's parents to participate in his/her experience on our site.
