LIABILITY
The student/legal guardian agrees that Angela Vital-Martowski, D/B/A The Dance Studio of Wilbraham, 35 Post Office Park, Wilbraham, MA, its agents, employees, and representatives shall not be held responsible for any injuries sustained by the customer while dancing on the premises or for The Dance Studio at any time off premises or off property.
PRIVACY/PHOTO/VIDEO RELEASE
The student and/or legal guardian agrees to be video recorded and/or photographed during classes, rehearsals, and performances for use by the The Dance Studio only, including websites and social media pages. No photos/videos will be placed on any social networking site belonging to staff/teachers without consent from the student/student’s guardian.
FALL - SPRING TUITION & FEES
The parent/student agrees to pay any tuition, costume and/or performance fees, to The Dance Studio, accrued during the above time frame. A student/parent must notify the studio prior to dropping a class/studio activity. If the studio is NOT notified before the first day of the payment due date, the student/parent will be responsible for the tuition of that pay period. This includes costumes and/or fees in full. All payments are due the first week of each payment period (no later than the 10th of each month). Any accounts more than 30 days past due will be subject to a $20 late fee. Payment can be made in person by cash or check or with a credit card online. Credit card payments are subject to a 3.75% processing fee.
SUMMER TUITION & FEES
The parent/student agrees to pay any fees due to The Dance Studio, its agents, employees, and/or representatives accrued in the time frame above. The parent/student acknowledges that the deposit paid at the time of registration is non-refundable and non-transferable, and guarantees a place enrolled in the summer activity. All remaining summer camp/workshop/intensive balances will be paid via cash or check directly to the teacher on the first day of the camp/workshop/intensive as part of our profit sharing program. Please see the description on the portal and/or the flyer for the specific camp/workshop/intensive to find the total balance.The parent/student agrees to pay tuition for summer evening classes and mini camp/activities at the time of registration. Payment can be made in person by cash or check or with a credit card online. Credit card payments are subject to a 3.75% processing fee. The parent/student acknowledges that registration for any summer camp/workshop/intensive, class, and evening mini camp/activity is not complete, and therefore space is not reserved, until the proper registration payment has been made. A student/parent must notify the studio 7 days prior to dropping a class/activity.
