PLEASE READ AND SIGN THAT YOU HAVE READ, UNDERSTOOD, AND AGREED TO THE FOLLOWING:
Studio D School of Dance and Festival Dance Camp will be called Studio D.
Payments, Late Fees, Auto-Pay: Tuition is charged annually. Annual tuition may be paid all at once; however, many choose to pay in 10 installments or monthly. We do not charge monthly tuition; our annual tuition is conveniently broken into payments to assist our families. If you choose to make monthly payments, these are due the first class of each month. Any payments made after the 15th of each month will automatically incur a $10 late fee. Payments can be made in the studio, mailed, online by credit card (processing fee will apply), or by enrolling in Auto-Pay. For Auto-Pay, you can opt-in online through your account.
Withdrawal and Refunds: An email must be submitted to withdraw from classes before the first month you would like to remove your student. Verbal withdrawals will not be accepted, and tuition will continue to be charged—no refunds or discounts for missed classes or school breaks.
Attendance and Care: If students cannot attend a class, please get in touch with the front desk or by email at studiodschoolofdance@gmail.com so that the instructor knows about the absence. The Studio is not responsible for providing students before or after-class care. Students should only be kept at the studio briefly before or after class and when waiting between classes. Make-up classes are available if you'd like.
Adult or Parent Responsibility: It is the responsibility of the adult or parent to be aware of all activities, such as class rescheduling, Parents Watch Week, and Studio closures. The school will post all such notices on the TV in the lobby and any upcoming closings at the front door. The adult or parent must regularly check these places and the email provided to ensure they are informed. It is also the responsibility of the adults or parents to notify the school of any email, address, or telephone number charges.
Costume Fees: Costumes are $95 per costume per class. Costume fees for the year-end recital will be due December 10th. No costume fees will be refunded after this date, even if a student withdraws from the school. Costumes are not required for the Musical Theatre portion of the recital.
Photo Release: The Studio is hereby granted permission to take photographs of the students to use in brochures, websites, posters, advertisements, and other promotional materials the studio creates. Consent is also hereby granted for the school to copyright such photographs. Select the option to opt-out if you do not wish for your child’s photo to be released.
Injuries: As the legal parent or guardian, I release and hold harmless Studio D School of Dance and its owners, operators, staff, contractors, or volunteers from any and all liability, claims, demands, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and the undersigned, while in or upon the premises or any premises under the control and supervision of Studio D School of Dance/Festival Dance Camp owners and operators, staff, contractors, or volunteers
or in route to or from any of said premises.
