WAIVER OF LIABILITY
INJURY, TUITION, AUTO-PAY & PHOTO RELEASE
COLORADO CONSERVATORY OF DANCE
1. Injury Waiver: I understand and agree that there is a possibility of injury in participating in dance classes, rehearsals, performances or related activities, and that I will not hold CCD, or any CCD faculty member, employee, board member or guest artist liable for any injury sustained or illness contracted while I/my child is participating at CCD. I exempt, release and indemnify CCD and its agents from any and all liability claims, demands, or causes of action whatsoever from any damage, loss or injury to the student, parent/guardian, family member, or personal property which may arise out of or in connection with participation in any CCD activity.
2. Tuition Policy: CCD's Conservatory season begins mid-August and runs 38 non-consecutive weeks as outlined by the school calendar. Tuition is calculated in 10 equal monthly installments through Gala. CCD’s Adult Program runs year-round with breaks for Thanksgiving (November 20-25), Winter Break (December 23-January 1), and Spring Break (March 25-31). Federal Holidays that CCD will be closed are: September 4, December 25, January 1, May 27, July 4 Tuition is due on the 1st of each month; monthly statements and account balances are available in the parent portal. Tuition received after the 5th of the month will result in a $35 late fee. Additional fees may be assessed each month the payment is overdue.
UNDER NO CIRCUMSTANCE WILL CCD REFUND OR CREDIT PAID TUITION. Exceptions may be made in the case of serious injury accompanied by a doctor's note.
To withdraw from classes, the Front Desk must be notified by the 15th of the month prior to withdrawing from classes (for example, notify the Front Desk by April 15th for May). Tuition installments can be paid by check, cash, or Visa/MasterCard. Monthly auto-payments can be set up through DanceStudio-Pro or by contacting the front desk. Returned checks will result in a $35 charge. Accounts past due on the 10th of the month may result in immediate suspension from classes until finances are settled. All accounts must be current for a student to participate in any CCD production, performance, or rehearsal.
3. Registration Fee Policy: There is an annual $35 registration fee. The registration fee for additional family members is $10 per student.
4. Auto-Pay Waiver: I understand if I enroll in Auto-Pay, my credit card will be charged the FULL BALANCE on my student's account on the 1st of each month. I can cancel at any time by accessing the DSP parent portal or by calling the CCD’s Front Desk. To avoid unwanted charges, please contact CCD by the 28th of the month to cancel Auto-Pay for the following 1st.
5. Photo Release: I give CCD permission to use images and likeness, and/or videos of my student(s) to promote the activities of CCD. I understand that the images and likeness may be used in print publications, presentations, reports, websites, social media and other media platforms. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.

ADDENDUM: COVID-19
COLORADO CONSERVATORY OF DANCE
While CCD has adopted comprehensive and aggressive protocols for cleaning, sanitization, and disinfecting of surfaces throughout its facility, I acknowledge the continued contagious nature of COVID-19 and other contagious diseases and viruses and voluntarily assume the risk that I and/or my children may be exposed to or infected by attending and participating in CCD classes, activities and performances. Such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, independent contractors, volunteers, and program participants and their families.
I release CCD, its instructors, independent contractors, staff, faculty and Board of Directors and all associates from liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses. I acknowledge receipt of, and understand and agree to, CCD policies and procedures. I understand that my student must have this release form signed before attending class.
I also agree not to enter the CCD facility if I, my child, or anyone in our immediate family is displaying any symptoms of the COVID-19 virus or other infectious illness as described in the organization’s health & safety protocols, and to either leave the facility or remove my child from the facility should any CCD faculty or staff member determine their physical condition represents potential exposure to the virus at any time you or they are inside the building. I understand I will not receive a refund or credit for classes or programs not attended.
This waiver covers the specific period from August 1, 2023 to July 31, 2024, and covers all programs, classes, rehearsals, performances, and all other activities inside the CCD facility at 3001 Industrial Ln. #12, Broomfield, Colorado, 80020 as well as all performance and community education venues. This waiver does not nullify nor replace the CCD general waiver of liability required of all dancers.
