Thank you for your continued support and trust in Dance With Danielle. As with the transmission of any communicable disease like a cold or the flu, you may be exposed to COVID-19, also known as the "Coronavirus", at any time or in any place. Be assured that we have always followed state and federal regulations to limit transmission of all diseases in our studio and continue to do so.

Despite our careful attention to sterilization, disinfection, and use of personal barriers, there is still a chance that you could be exposed to an illness in the studio, just as you might be at your gym, grocery store or favorite restaurant. Social distancing has reduced the transmission of Coronavirus. Although we have taken several measures to provide social distancing and contactless participation, please know that participation in Dance With Danielle instructional classes is at your own risk.

I accept the risks and consent to allow my child to participate. I also agree to adhere to all of the Dance With Danielle safety guidelines and measures to help stop the transmission of COVID-19.

I HEREBY WAIVE, RELEASE, AND DISCHARGE Dance With Danielle and all divisions there of any and all liability and responsibility for injuries, sickness, pandemics, accidents, natural disasters and/or acts of God incurred during my childs/children's participation in and/or instruction of camps, intensives, private instruction, choreography, rehearsals, recitals, studio arranged events, or any activity they may participate in with Dance With Danielle



A special note - Dance With Danielle has the right to stand by or change our policies due to unforeseen circumstances such as natural disasters, pandemics, or other government mandated procedures.
