Medical Agreement

I hereby acknowledge and agree to the following:

I understand and acknowledge that participation in dance activities, whether online or in-person, involves the risk of injury. By participating in these activities, I voluntarily assume all risks associated with the activity, whether known or unknown to me.

I release DanzStar, LLC and its directors, officers, volunteers, representatives, and agents from any and all liabilities, claims, or actions, including but not limited to those resulting from my own negligence. I also understand that DanzStar, LLC is not responsible for any errors, omissions, or failures to act by any third party involved in the activities.

In the event that I, my family, or I cause damage to any equipment or facilities due to negligence or recklessness, I agree to be held liable for any associated costs.

I further agree to indemnify and hold harmless DanzStar, LLC against any claims, suits, or actions for liability, damages, or compensation, brought by me or anyone on my behalf, including attorney fees and related costs.

I confirm that this agreement has been entered into voluntarily and without coercion, and I fully understand its terms. I acknowledge that this agreement is clear and unambiguous and that I consent to it of my own free will.

In the event that any provision of this agreement is deemed to be invalid or unenforceable, the remainder of the agreement shall remain in effect. If any provision can be modified to make it valid and enforceable, it shall be interpreted as so modified.

By signing this electronically, I confirm that I have read and understood the liability release waiver and that I am at least eighteen (18) years old. I acknowledge the risks involved and give my voluntary consent to be bound by this waiver. I also agree to the use of my electronic initials in place of an original signature.
