I the (student/guardian/parent) recognize and acknowledge that there are risks of physical injury to participants of Woodland Dance Center (WDC) and I agree to assume full risk of any injuries, damages, or losses which my child or I may sustain as a result of participating in any and all activities with Woodland Dance Center.
In the event of an emergency I authorize Woodland Dance Center teachers to secure from any licensed hospital, physical and/or trained medical personnel any treatment deemed necessary for your child’s/children’s immediate care. I also agree that I, the parent/guardian/student, will be responsible for any and all medical services rendered.
I understand that WDC follows the Woodland School District’s calendar for holidays and spring break and that there will be no dance classes during these breaks unless otherwise specified. There will also be no make-up classes for these breaks. I also understand that if I/my child miss a class the teacher and/or WDC is under no obligation to make it up to me/my child/children, or reimburse me in any way.
I also understand that attendance is important for the entire class to progress. Multiple unexcused absences may result in removal from class without refund.
I agree that my child/children will be at classes on time in clothes that follow the dress code. (Example: Ballet black or pink color leotard, pink tights, ballet shoes with hair pulled back off the neck.) If my child/children is more than 15 minutes late they will not dance in class, but will participate by taking notes, and observing the teacher and fellow students.
I agree to pay full tuition the first week of each month September 2022-June 2023, or starting at time of enrollment if enrolled in between these time periods. If this payment is not received by the 7th of each month I agree to pay a $10 late fee per student. Monthly tuition is non-refundable. I agree to give a minimum of 2 weeks’ notice if my child withdraws from class.
I understand that pictures will be taken during classes, rehearsals and performances of WDC students. These pictures will be used for publicity on social media, and/or as decorations at WDC. To opt out of pictures of my child/ren being used for any or all of these purposes, I will submit a written and signed letter of request to WDC's board of directors.
