I. TUITION FEES
Monthly tuition fees are due on the 1st of each month. Tuition is per month not per class. After a 5-day grace period, any payment not received is subject to a $10.00 late fee. If there is not payment by the 7th of the month, your card will be charged that months full balance inclusive of the $10 late fee. The tuition fees are the same every month, including the months shortened by holidays. Tuition remains the same regardless of absences and whether it is a long (5-weeks) month or a short (3-weeks) month. Tuition is based off a 10-month annual year, beginning in August and ending in June. Please read the following payment options below and circle your preferred payment option. Please sign the Credit Card Authorization form and provide a copy of your credit/debit along with a copy of your driver’s license for our records.
Autorenewal Registration: At the conclusion of each dance season, all current students are automatically enrolled in the next upcoming dance season. If your prior dance season bill is not current at the end of the season, you will not be able to register for the upcoming dance season. During this process, administrative, front desk, and teaching staff will determine the most optimal class for each student. If you need to change your class due to scheduling conflicts, or drop your enrollment altogether, you must contact the Director after annual recitals are completed. Registration Fees for the next dance season will be run via autopay on July 1.
Roll-over Registration: The season is for 10-months, but due to the date of the recital, the dance class registration may roll-over to the following month and will be month to month until a new dance agreement is signed.
Payment Options:
1. You may pay by Credit/Debit Card. The account holder is responsible for providing a copy of their credit/debit card and their driver’s license/ID for our records. The account holder will also sign a credit authorization form. The credit card authorization will remain in effect until cancelled. You may cancel this authorization by written notice to the Owner within thirty (30) days before upcoming tuition payment.
*IMPORTANT*: Upon enrollment, you must provide a credit card, debit card, or checking account number to be placed on file in Intencity, LLC’s autopay tuition system. There is 3.5% surcharge added to all autopay and credit/debit transactions. Tuition may be paid via check or cash before the first of each month if you do not wish to utilize the autopay system. Nevertheless, a credit/debit/checking account must be placed on file with Intencity, LLC to ensure tuition is paid in full.
2. You may pay by Check. (There will be a $40 fee incurred for each check that is returned.) In the event that any check is returned unpaid for any reason, the Owner may place a stop transfer order against all shares issued a result of such check or order; and the Owner may take appropriate action at any time.

II. DANCESTUDIO-PRO LOGIN
All parents are given a DanceStudio-Pro login (DSP). This login will give you access to register for classes, make online payments, book private lessons, view your calendar and view your account. Your portal is utilized for all communication. Please monitor your account frequently to ensure you don’t miss any payment dates, important updates, and any emergency alerts.
III. ENROLLMENT FEE
An annual non-refundable $45 Enrollment Fee per student (or per 1st member of the family) to cover administrative costs is required yearly at the time of registration. Additional family members are charged the annual enrollment fee at a discounted rate as follows: $25 for 2nd member of the family, $15 for the third member and all additional members of the family.
Sibling Discounts: You’ll receive 10% off total tuition for the second sibling and each sibling thereafter (only tuition is included with this discount).
**Active Military, First Responders, Nurses, Firefighters, Clark County School Teacher’s, and Police will receive 15% off total tuition (only tuition is included with this discount).
*Only ONE Discount may be allowed per family. You may not combine discounts.
*Please notify the Director upon signing this Agreement if any of the above-referenced discounts apply. Discounts are not automatically applied at checkout.

B. STUDENT PLACEMENT AND PROGRESSION
Our professional staff at Intencity Dance are trained to teach the art of dance. Our instructors are professionals who will determine your child’s educational decisions regarding their dance training. New students are placed by age, grade, number of years’ experience, type of training and teacher’s discretion. We do our best to place each dancer on the background you provide for us. If a student is placed incorrectly, they will be moved during the first few classes. Not all dancers progress at the same pace. Many factors such as physical growth, learning speed, coordination, number of classes, natural ability, etc., play a primary role. There are great physical and emotional risks involved with a dancer progressing too quickly. Please allow us to place your child where we feel they can enjoy dancing, learn correctly and make excellent progress. Any questions or concerns regarding your child’s class placement and progress are welcomed.
Please contact the Director to schedule an appointment to address any concerns. C. CLASS INFORMATION
Each class offered at Intencity Dance will be taught by a qualified instructor. Student assistants will be present in classes when deemed necessary by the director. Please stay with your child until their class begins. If anyone other than the parent or guardian will be picking up the dancer, inform the teacher/office ahead of time. A short water break will be taken during the two-hour classes. Please bring a water bottle. Students are not allowed to bring food or drink into the classroom. Cell phones must be turned off during class. If there is an emergency, please call the studio office. T-shirts are only permitted for Hip-Hop class and are not allowed in any other dance class or dance room. Dancers with T-shirts will be asked to leave class and sit in the lobby. Instructors need to see the movement and body position of the dancer to properly instruct.

F. PRIVATE LESSON
The private lesson rates may vary based upon the Owner’s discretion. The private lesson rates are reflected below:

§ Room Rentals require a Rental Agreement to be signed before being rented.
H. FEES
You’ll receive a due date and price list upon signing the contracts, which will be
handed back to you by the Owner.
I. PRODUCTION FEE
Production Fee include the shipping and handling charges, administrative costs of ordering, production charges, theater rental, special equipment rental, props, sets, lighting, sound, additional recital administrative costs, programs, and printing costs. The fee is $120.00 and is considered due at time of registration. The deadline is November 1st. After that date there is a late costume fee of $40 (if we can accomplish this with the costume company). Any costume not picked up by May 1st or June 1st, contingent upon if the recital date occurs in May or June, will become studio property. Costume exchanges must be within the first week of issuance. All fees must be paid by June 1st to be eligible to perform. No refunds will be issued for any reason.
Costume Deposit and Payments
A mandatory costume deposit of $35 for each costume is due by October 20, by 6:30 PM. The balance for costume payments is due during costume measuring week with a grace period ending December 8, by 6:30 PM. Costume prices range on average between $65.00 and $80.00 each. These dates and times are final. Please do not ask for exceptions. A costume will not be ordered for your dancer if your costume is not paid in full by December 8. Extensions are not available. Checks for costume payments must be postdated for December 8th and remitted to the Director during Measurement Week for your convenience.

K. RECITALS
Recital and recital rehearsals are tentatively scheduled for May or June. Please be
open and available between 10 am – 6 pm.
We regret we are unable to accept any special requests for days or times. Please
note, you may be asked to provide specific color tights and/or dance shoes for the recital. We are instilling the value of a polished, professional performance which requires regular attendance, dedicated practice and full-dress rehearsal participation. Therefore, our dress rehearsals are mandatory, and our dancers may not participate in dress rehearsal only, as this proves detrimental to the entire group and final presentation.
L. SpecialPrograms
Competitive Dance Team (By Audition Only)
Intencity Dance offers students an opportunity to audition for our competition dance team. The dance team travels to competitions and dance workshops. They also perform in our community at festivals and events. Auditions are held each year in May and August. Please contact the office if you're interested in auditioning for our competition dance team.
Requirements: Dancers must have prior dance experience. Dancers must also be at least five years of age. Competition dance team members must enroll in required classes, see director for more details. Additional performance and travel requirements are determined by the director and vary each season.
M. REFERRALS
You will receive a $75 credit to your account for referring a new student to the
studio. Your credit will appear after they register and pay their first month’s tuition.
N. ABSENCES
There are no refunds or credits given if a student is absent from class. You can
make-up classes within the same month (30 days) in another class of the same level. If your usual instructor is absent, another qualified member of our staff will be available to substitute the lesson. If a substitute cannot be provided, either the class will be rescheduled by the instructor or you will receive a per lesson credit towards your next month’s tuition based on the Owner’s discretion.
O. TARDINESS
Student must be on time for each scheduled class at the proper time of arrival. If
the Student is 10 minutes past their scheduled class time, the Student may not be allowed in class based upon the Teacher’s discretion. If there are any questions or concerns and a Parent would like to meet with a Staff Member, the Parent must then email the Owner to make an appointment. Remember all Directors, Staff Members, and Guest Artists strictly enforce all policies.
P. CLASS ETIQUETTE
All students must present themselves always with dignity, pride, and respect not
only for others, but also for themselves in and out of the classroom setting. Each Student must arrive on time to their appropriate class with all hair out of their face, no chewing gum, no talking in class unless spoken to by the Instructor, Director, or Choreographer. Always remember that first and foremost dance is a discipline that should be taken seriously, always.
Your discipline, hard work, and dedication will determine your skill level. The goals set forth by you will determine how you will improve in the way desired. Teachers can only do so much then it’s up to the students and parents to be of assistance in this matter.
Q. TERMINATION OF LESSONS
Thirty (30) day notice is required to discontinue any lessons. Withdrawal must be in written form and must be provided in person. Withdrawal will not be accepted over the phone or via electronic communication. You will be considered a student and billed accordingly unless a withdrawal form is completed. To withdraw from lessons a parent or adult student must:

2. Complete and sign a withdrawal form provided by IntenCity Dance.’’ 3. Return signed withdrawal form to the Owner in person.
R. CANCELLATION OF PRIVATES
You may cancel and/or reschedule any privates within 24 hours. You may cancel
by contacting the Front Desk, Instructor, Director, or Owner. Same day cancellation is a 50% charge. No call / No show is 100% charge.
S. HOLIDAYS
Our lesson schedule does not follow the school calendar. Lessons will not be held
on: New Year’s Eve, New Year’s Day, Memorial Day, 4th of July, Labor Day, Thanksgiving Eve, Thanksgiving Day, Christmas Eve, and Christmas Day.
For School vacations such as, Christmas break and Spring break, regular classes will be closed. Please be prepared all company rehearsals will continue over such breaks.
T. GENERAL EXPECTATIONS
1. For the student’s safety, no one is allowed in any studio without a teacher or
without permission from the Owner.
2. No food, drink, or gum in the classrooms. Only water is allowed.
3. No running or horseplay anywhere in the building.
4. No street shoes on the dance floors.
5. Parents must notify the studio if their child will not be attending class.
6. Do not leave anyone, especially children, unattended in waiting areas.
7. Arrive on time, or early, and prepared for classes. Please allow a few minutes for
your child to adjust from their previous activity to the dance studio. We respect that everyone has a schedule. Please respect ours. Please make sure your child is there 15 minutes prior to the start of class to stretch their muscles.
8. Missed lessons should be made up in a same or lower-level class. (Unless otherwise specified, on an individual basis.) Classes do not have to be made up in the same style of dance if one is not available. Experimentation is encouraged. Please contact the office when making up a class.

U. GENERAL STUDENT EXPECTATIONS
1. Students will help their parents out by being ready to go to class by the time
parents are ready to leave their home.
2. Students show respect for themselves and others by being dressed appropriately
for class, such as:
A. No jewelry, other than small earrings
B. Shoe strings should either be tucked in or tied in a knot and cut off.
C. Please do not wear underwear under girls’ leotards when wearing tights.
(Traditionally tights act as underwear. Wear underwear when not wearing
tights in the warm weather.)
D. Student’s hair is to be pulled up off the neck. For 6-year old and under, a
bun is preferred.
3. Students show respect for others by:
A. Keeping their hands to themselves during class.
B. Waiting quietly for others to have a turn.
C. Waiting until the music is finished before entering the classroom if they
are late to class.
D. Not borrowing items, cash or other valuables without permission from the
Owner of the item(s).
4. Dancers show respect for their teacher and the art form they are learning by:
A. Being properly dressed and ready for class on time.
B. Listening when the teacher speaks.
C. When sitting on the floor, sitting criss cross style with back up straight.
D. Being prepared for their turn.
E. Always asking before leaving the room for any reason.
F. Applauding their teacher at the end of class.
G. Do not bring in gum, drinks other than water, or food into the classrooms.
H. Never hanging on the barres.
I. Discarding trash in the proper places.
J. NEVER touching or hitting the mirrors, a very important safety issue.
V. GENERAL EXPECTATIONS OF PARENTS
1. Parents show respect for the dance class, teacher, and studio by:
A. Knocking before entering the classroom, if a class is in progress.
B. Picking students up promptly after class.
C. Making appointments to meet with the teacher if a conversation is
expected to exceed more than a few minutes.
Please make sure that your child goes to the bathroom right before you leave home. This will ensure that your child will not miss out on valuable lesson time while they’re in the bathroom.
Please give your child the space and freedom to explore dance on his/her own. If your child has problems separating from you, that is usually a good sign that your child should most likely wait 6 months to a year before they enroll in dance lessons.
Proper hygiene is essential in all aspects of life. It is very hard to approach a child or parent about a lack of cleanliness or a foul body odor. However, in a dance studio where activities induce sweat and children are moving through the space, odors can become a terrible distraction and an embarrassment for the child, should a classmate be insensitive and say something out loud. To avoid these problems, please be sure all dancewear is laundered after each wearing; hair should be clean and pulled back properly, and deodorant should be a consideration for all students age 7 and up for dance class. Please, NO PERFUME prior to class as this will cause a terrible odor, as well as aggravate allergies of other students.
2. Waiting Room Etiquette
Intencity Dance was founded on family, and this is an important part of what we do. Although we are a “child friendly” environment, we are still a business. All children should be supervised while in the building. Do not allow them to play with the chairs, divider wall, doors, blinds, etc. Siblings are welcome but activities should be quiet and not disruptive to class, office, or others waiting in the lobby area. Please keep conversations age appropriate for children's ears and be respectful of others waiting. We appreciate your assistance in keeping our facility clean and pest free. You will be charged if students are not picked up in a timely manner.
3. Classroom Etiquette & Viewing
We all want your child to enjoy dance class, but it’s also important that the class remains in control. Students are expected to behave respectfully to fellow students as well as the instructor. We as teachers understand that some days are better than others, however, students who are excessively disruptive during class will be asked to either sit in the classroom or leave the room for the duration of the class. The instructor will fill out a disciplinary form and have parents sign when necessary. Class viewing is at the discretion of the director. The studio doors will be closed for the majority of classes. Remember you are a “stranger” to the other students in the classroom and could be a distraction keeping them from fully participating in class. The director may invite parents to watch part of the class on certain days. Parent observation weeks will be scheduled at the discretion of the director. Notification will be sent home with the dancers prior to parent observation classes.
W. DRESS CODE
Girls
COMBO – The combo classes require leotard and tights. Underwear is not necessary when tights are worn. Underwear is necessary when tights are not worn. Pink ballet shoes and black tap shoes are required.
BALLET – Black leotards are required. Ballet pink or theatrical pink tights are required and pink ballet shoes. Teens and adults have permission to wear solid colored unitards. T-shirts and other cover-ups are not allowed. Tube socks are not allowed. Shorts are not allowed. A sweater may be worn at the beginning of class to warm-up in during the cold winter months.
TAP, JAZZ, LYRICAL, PRE-DANCE, & ALL OTHER – Modest dancewear such as leotards, tights, sweatpants, and skirts. Street clothes and/or baggy clothes are NOT permitted. Make all dance apparel purchases from a retail store which stocks those brands that are acceptable for the proper and modest dress for dance studies. Off-brands often do not fit properly and can be a source of pain and frustration. A cover-up should be worn entering and leaving the studio no matter what the season or weather. Never wear dance shoes outside.

HIP-HOP – Baggy clothes are permitted in this class. Absolutely no jeans!
Boys
Same rules apply, No Baggy Sportswear, only dance attire. Black tights, non-baggy shorts, or sweatpants, white or black ballet shoes, white socks, Muscle T-shirt. No baggy shirts.

X. DISCLAIMERS
Release of Liability
As the legal parent or guardian, I release and hold harmless Intencity Dance, its Owners and Operators from all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Intencity Dance.
Medical Emergency
The undersigned gives permission to Intencity LLC, its Owners and Operators to seek medical
treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.
Payment Information
Tuition is due by the first of each month. If accounts are paid after the 5th of the month, there will be a
$10.00 late fee applied to the account balance. There is a $40.00 returned check charge for any checks returned by the bank. Please see the front desk to make payment arrangements.
Y. AcknowledgementofUnderstanding
The Participant and Parent(s) or Guardian(s) have read this waiver of liability and indemnification agreement and fully understand its terms. The Parent(s) or Guardian(s) understand that Participant is giving up substantial rights, including the right to compensation for injury resulting from ordinary negligence. The Participant and Parent(s) or Guardian(s) acknowledge that you are acknowledging the agreement freely and voluntarily and understand your reading of this disclaimer is a complete and unconditional release of liability to the greatest extent allowed by law. In acknowledging this waiver of liability as Parent or Guardian, you agree that you are consenting to the Participant’s participation at Intencity LLC dba Intencity Dance and acknowledge that you agree that all risks, including that of ordinary negligence, whether known or unknown, are expressly assumed by the Participant and Parent or Guardian and all claims, whether known or unknown, are expressly waived in advance.
IntenCity Dance reserves the right to terminate lessons to any student without notice. In such a case a refund for unused lessons will not be given IntenCity Dance reserves the right to change policies as needed. Students will be notified of changes when they occur.

Default
• Please read, initial and sign in the space provided below:
If you’re in default more than forty-five (45) days you will not be permitted back into the studio. All payment arrangements must be made in advance on your account, before your balance reaches two (2) times your tuition. These arrangements are solely situational. You will only be allowed twice within the year before Participant is suspended. You’re expected to stay current on all financial obligations. These expenses include but are not limited to costumes, conventions, competitions, tuition, music, room rentals, travel and recital fees. If your dancers account is in default 30 days before competition, you won’t be allowed to compete.
Non-Disclosure Agreement
I, , hereby agree to not disclose any information obtained in the dance year. All financial adjustments, scholarships, monetary payments both drafted or verbal will not be discussed through third parties or within the confines of IntenCity. For this would be a breach of contract with IntenCity LLC and will be handled as such at Owner’s discretion.
Tax Disclosure
All taxable items upon purchase will incur State Sales Tax. All credit card transactions will be charged a separate fee. These are non-negotiable fees and must be paid upon purchase.
We, the staff at IntenCity Dance , recognize our obligation to make sure our students and their parents are aware of the risks and hazards involved in the sport of dance. By signing this waiver, you release, IntenCity Dance and all its employees from all claims on account of any injury which may be sustained by you or your child while attending any dance class, event, or outside performance. In signing this waiver, you also acknowledge your responsibility in paying monthly tuition, any associated costumes, entry fees for performances or competitions, and all other communicated costs involved. You also affirm you now have and will continue to carry proper primary medical, health, hospitalization, and accident insurance, which you consider adequate for the protection of both your child and IntenCity Dance . I give permission to take photographs/videography of the students to use in brochures, web sites, advertisements and other promotional materials the studio creates.
