Innovative Dance Company Liability Waiver and Acknowledgment of Risk:
Assumption of Risk
In consideration of my child's/children's acceptance into Innovative Dance Company, I understand that my child's participation is entirely voluntary. I understand that this sport, like many other sports, poses a risk for accidental injury that may be life/limb threatening.
Release of Liability
I hereby, forever release Innovative Dance Company and all personnel connected with Innovative from liability for any and all damages or injuries suffered by my child/children in connection with his/her participation in the program which includes both inside and outside of the building and at any location. I understand and agree that I am solely responsible for any and all medical expenses that may arise due to an injury.
Medical Emergencies
If myself or the emergency contact cannot be contacted in the event of an injury or medical emergency, I hereby authorize an instructor of Innovative Dance Company to obtain emergency medical care from any authorized emergency medical agent and I accept full responsibility for any and all costs from medical treatment received.
Registration Fee
Registration is due at the time of sign up and will be due every year there-after. Registration will be due each September regardless of the month that you initially signed up in and paid the fee. Registration is nonrefundable or transferable. It can not be used as a lesson payment. If an unpaid within 24 hours, dancer will be dropped from class.
Attire
A leotard, unitard, leggings and tank are recommended. All dancers must wear form fitted, athletic clothing. NO baggy shirts or jeans.
We recommend hair being pulled back out of face.
All dancers should have required dance shoes.
Drop off/Pick Up
Please do not drop off your child more than 5 minutes early for their class. It is important that you arrive on time to pick your child up on time.
Photography/Video
I give permission to Innovative Dance Company instructor's to take photographs and/or videos with audio of my child(ren) during class actives including but not limited to: Class time, parties, special events.
Withdrawal Policy
To withdraw from the program, notice is required by the by the 15th of the month to withdraw before the next billing cycle.
Payment Policy
Tuition is due on the 5th of each month. You may not pay per lesson.
I understand and agree that Innovative Dance Company may charge my account listed all costs agreed upon when acknowledging these policies. If tuition is not paid by the second lesson of the month, participation will not be permitted until the account is current.
Monthly Tuition and/or registration is nonrefundable once paid.
I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of Innovative Dance Company classes, rehearsals, competitions, performances, or activities. I also exempt, release, and indemnify Innovative Dance Company, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Innovative Dance Company. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Innovative Dance Company, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.
Permission is granted Innovative Dance Company to use photographs of students for publicity purposes.
I have read, understood and agree to be bound by the above statement
