Conditions of Enrollment:
1. All students are to arrive on time. Please allow time to use the restroom before class begins.
2. Students should wear proper dance attire, and have their hair neatly pulled back away from their face. Proper dance shoes are required, unless otherwise instructed. If not dressed appropriately, students will be asked to sit out of class and will not be reimbursed. Repeated failure to dress appropriately may result in termination of enrollment.
3. B Inspired Dance Studio understands when life happens and you can’t make it to class. We ask that you give a courtesy call or email 24 hours in advance to notify us of missed classes. Should we receive notification before the class has begun, you will not be charged for your missed class. Students will be held financially responsible for all “no call, no shows”. If missing payment for more than 4 classes, students are required to sit out and not participate until payments are made. Late fees will be applied to all outstanding balances. - THIS DOES NOT APPLY TO SUMMER CLASSES. Summer Classes are paid for at the time of registration, will not be prorated or refunded. Registration fees are collected at the time of fall and spring registration, and will not be prorated or refunded.
4. Cell phone use is not permitted during class time.
5. If participating in the end of the year recital, purchase of a costume is required in January 2024. Any costumes purchased after due date will acquire a late fee.
6. Students must wait for rides inside the studio doors.
7. No food or drinks (other than water) are permitted in the studios. Students are not permitted to chew gum. Any student damaging or destroying studio property will be promptly dismissed. Students are not permitted to smoke.
8. Proper language and good behavior are required at all times. A disruptive student will be asked to leave class and/or B Inspired Dance Studio and a parent/guardian will be notified.
9. Should a student withdraw from a class, they will be charged for the remainder of that month. Starting the following month, they will no longer be held financially responsible for future tuition.
10. I understand that should I not make payments on time, my credit card held on file will be charged for the outstanding balance and will include a processing fee.
11. I hereby give B Inspired Dance Studio and its photographers, their assigns, successors in interest, legal representatives, and heirs the irrevocable right to use any photographs taken for B Inspired Dance Studio of any of my children or family in all forms and in all media in all manners, without any restriction to changes or alterations. I agree that the photographer will hold all copyrights to these photographs.
11. Indicated in my online registration are any health problems or conditions of which the studio should be aware of. I understand that the risk of injury is inherent in any physical activity and I, on behalf of myself and my child, knowingly and voluntarily accept that risk. I, the undersigned for myself, my heirs, administrators, and executors hereby waive and release B Inspired Dance Studio, LLC. and its staff from any and all claims or damages of any kind arising out of my child’s participation in the exercise and/or dance program of B Inspired Dance Studio, LLC. I further certify that the aforementioned student is in proper physical condition to participate in the exercise/dance program and that he/she has been examined by a licensed physician and found to be in proper physical condition to participate in said program. I, the undersigned do hereby authorize B Inspired Dance Studio, LLC. or designated agents (being teachers or administrators employed by B Inspired Dance Studio) to obtain medical treatment for my said child in emergency situations where I cannot be reached in time to authorize the treating physician to provide such emergency medical services. I understand that I am responsible for any medical expenses. This authority includes the power authorizing any and all treatment deemed necessary under the circumstances by a licensed physician. This power is in essence a power of attorney and shall remain for one year from the date signed below.

Thank you for your continued support and trust in B Inspired Dance Studio. As with the transmission of any communicable disease like a cold or the flu, you may be exposed to COVID-19, also known as the “Coronavirus”, at any time or in any place. Be assured that we have always followed state and federal regulations to limit transmission of all diseases in our studio and continue to do so.
Despite our careful attention to sterilization, disinfection, and use of personal barriers, there is still a chance that you could be exposed to an illness in the studio, just as you might be at your gym, grocery store or favorite restaurant. Social distancing has reduced the transmission of COVID-19. Although we have taken several measures to provide social distancing and contactless participation, please know that participation in B Inspired Dance Studio instructional classes is at your own risk.
Although exposure is unlikely, you accept the risk and consent to participation.

I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin hereby release and hold harmless B Inspired Dance Studio, their officers, officials, agents, and/or employees, and other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct any event, with respect to any and all illness, disability, death, or loss or damage to person or property, whether arising from the negligence of releases or otherwise, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY.
