Shoreline Ballet, Inc. Liability Waiver and Acknowledgment of Risk:
READ AND SIGN BELOW REGISTRATION IS INCOMPLETE WITHOUT SIGNATURE AND MUST BE COMPLETED BEFORE CLASS
I understand and agree that participating in any dance class, workshop, rehearsal or performance carries with it potential hazards and that there is a possibility of physical injury or death as a result of my participation in these events. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any Shoreline Ballet classes, rehearsals, performances, or activities. I also discharge, exempt, release, and/or otherwise indemnify Shoreline Ballet, Inc., its owners, agents, volunteers, assistants, employees, guest artists, faculty members, students and any other affiliated organizations, their employees and associated personnel from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or my or their property which may arise out of or in connection with participation in any classes, performances, workshops or any other activities conducted by Shoreline Ballet, Inc. or an affiliated organization.
If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.
Shoreline Ballet, Inc. is not responsible for the loss of my or my children’s personal property.
Throughout the year, there may be times when Shoreline Ballet, Inc. staff may take photographs of students, audiotape/videotape students, or interview students for use on its website or in advertisements, flyers or other promotional material related to Shoreline Ballet, Inc. By signing this document I give permission to Shoreline Ballet, Inc., its officers, employees and agents to use my or my child’s photograph, videotaped image or quotes for the purposes mentioned above. I understand and agree that Shoreline Ballet, Inc. may use photographs or videotaped images in subsequent school years unless I revoke this authorization by notifying the officers of Shoreline Ballet, Inc. in writing.
I agree to be responsible for reading studio correspondence, respecting studio policies, respecting dance instructors and other students and agree that the failure of either myself or my child to abide by the rules and policies established by Shoreline Ballet, Inc., its officers, employees, instructors or agents may be grounds for dismissal of me or my child from any programs in which I or they have been enrolled.
I hereby acknowledge that I have read the Shoreline Ballet Waiver and agree to adhere to the polices and rules set forth therein accordingly.
I have read, understood and agree to be bound by the above statement.
