Waiver of Liability
I am aware The Dance Studio takes every precaution to assure my child's safety. However, if an accident should occur and I cannot be notified, I authorize The Dance Studio to seek emergency treatment at my own expense. I will not hold The Dance Studio or any staff members liable.
I give The Dance Studio permission to use my child's dance pictures / videos for promotional purposes.
