Participant Waiver

I hereby request that the above participant be accepted into all of the dance and performance activities associated with Coastal Dance Academy, Inc. I, the undersigned, hereby acknowledge that on behalf of myself and the participant named above, I have informed myself of the various dangers and risks associated with or inherent in dance.
Participant to Indemnify Coastal Dance Academy, Inc. In consideration of the fees to be paid and the instruction provided, I hereby agree to indemnify and hold harmless, Coastal Dance Academy, Inc. and its officers, instructors, and volunteers from claims, demands and causes of action howsoever arising out of any injury including death, loss, or damage to the participant named above while participating in the Coastal Dance Academy, Inc. program activities and any instructions.
Participant to Waive Claims Against Coastal Dance Academy, Inc. On behalf of myself, my heirs and assigns, I hereby agree to release, save and forever discharge Coastal Dance Academy, Inc. and its officers, instructors, and volunteers from claims, costs, causes of action, or demands, howsoever arising out of any injury, including death, loss, or damage, as a result of participation in the CDA, Inc. dance program and/or activities by the participant named above. I have read and understood the policies of Coastal Dance Academy, Inc. and I agree to its terms and conditions.
