I hereby give permission for images of my child or myself, captured during regular classes, rehearsals, performances and special activities through video, photo and digital camera, to be used by Coastal Dance Academy, Inc. in its promotional materials, newsletters, websites, publications and all forms of advertising (facebook, television, radio, newspaper, magazines etc.) and I hereby waive any rights of compensation or ownership thereto.
