Consent to Electronic Signature
Please read below carefully. If you do not consent to electronic signature, please quit registration process and come into our location for paper registration.
By clicking the box below, you are consenting to electronic signature in regards to Bold Moves Dance Company registration and participation. Electronic registration also consents to communication electronically by allowing Bold Moves Dance Company to send you notices, forms, and other notifications in an electronic format. Clicking below also consents that your electronic signature is equivalent to written signature.
BMDC Handbook
Bold Moves Dance Company requires every parent, dancer, and guardian to read and understand the rules and policies listed in the Bold Moves Dance Company Handbook. The Bold Moves Dance Company Handbook can be found on the Handbook tab of the website at www.boldmovesdancecompany.com
By clicking below, you agree you have received, read, and understand the rules and policies that were provided in the Handbook. By clicking below, you also acknowledge parents, guardians, and dancer will comply with all rules and policies set forth by Bold Moves Dance Company and any failure to comply will result in suspension or expulsion of the dancer from Bold Moves Dance Company and activities.
Payment Polices
I agree to provide payment for all services rendered by credit/debit car, check, or cash. Regular season monthly tuition is due on the 1st of every month and is considered late on the 11th of each month. If payment is not made by the 10th a late fee of $10 is added to your account. If the payment is still not paid by the end of the month an additional $10 late fee will be applied. Registration fees, tuition, costume deposits, competition fees, recital fees, etc. are all NON REFUNDABLE.
Low Enrollment Policy
Bold Moves Dance Company holds the right to cancel classes based on low enrollment. If in the event of a canceled class, a dancer may transfer into another class in the corresponding level and age range appropriate, with approval of teacher.
Pick Up/Drop Off/Unattended students or children
Common areas of Bold Moves Dance Company outside of the dance studio Studios A & B are not supervised by staff at all times. A parent or guardian must supervise student and siblings with student at all times. Students should not arrive more than 10 minutes before scheduled class; students also should be picked up no later than 5 minutes following class. Any early arrival or late pick up more than 10 minutes will result in a babysitting fee of $15 for every 10 minutes prior or following the 10 minute window allowed.
Photo and Video Acknowledgement and Release
Permission is given to Bold Moves Dance Company to use and publish for editorial, trade, advertisement, and other promotional purpose all photographic, video, and digital images of my child, including the child's name in conjunction with Studio and School Name. All parents who do not wish their child's image or name to be used in this manner must submit so in writing to Bold Moves Dance Company.
Photography Release Policy
By clicking below, permission is given to Bold Moves Dance Company to use and publish for editorial, trade, advertisement, and other promotional purpose all photographic, video, and digital images of my child, including the child's name in conjunction with Studio and School Name. All parents who do not wish their child's image or name to be used in this manner must submit so in writing to Bold Moves Dance Company
Acknowledgment and Release
By clicking the box below, you have read and understand the following statements:
1- I recognize and understand the risks of physical injury inherent to dance and dance training and I fully assume those risks. I hereby release Bold Moves Dance Company, its affiliates dance studios, event sponsors, employees and dance teachers from all liability for injuries sustained or illnesses contracted while attending or participating in any dance classes, rehearsals, workshops, or performances. I agree to indemnify, defend, and hold harmless Bold Moves Dance Company, its affiliate dance studios, event sponsors, employees and dance teachers for liabilities, costs and judgments arising from acts of omissions committed by me or my child which result in injury or damage to any person or property.
2- I understand and agree that it is my sole responsibility to safeguard my personal property while attending or participating in any classes, rehearsals, workshops, or performances. I hereby release Bold Moves Dance Company its affiliate dance studios, event sponsors, employees and dance teachers from all liability for loss or damage to my personal property while attending or participating in classes, rehearsals, workshops, or performances. I also agree to abide by any rules, regulations and policies set forth by Bold Moves Dance Company.
3-In case of physical injury or medical emergency, I hereby authorize Bold Moves Dance Company to make necessary arrangements to transport myself or my child to a medical treatment facility as necessary. All such transportation and medical treatment will be at my sole cost and expense. In extreme emergency, or if my child is under 18 years of age, I understand that Bold Moves Dance Company will attempt to notify the person(s) I have named below as my emergency contact(s) of my condition and how to reach me.
4-In signing this Release, I acknowledge and represent that I have fully informed myself of the content of the waiver and hold harmless agreement by reading it before I sign it, and I understand that I sign this document as my own free act and deed; no oral representations, statements, or inducements, apart from the written statement, have been made. I further state that I am at least eighteen (18) years of age and am fully competent to sign this agreement; and that I execute this release for full, adequate, and complete consideration fully intending to be bound by the same. I further state that there are no health-related reasons or problems which preclude or restrict my or my child's participation in this activity, and that I will pay any medical costs that may be attendant as a result of injury to me or my child.
