Agreement Statement 2023-2024

Agreement Statements:
1) Parent/Guardian gives consent for the student to be photographed without compensation, for use in media, ourwebsite, publications, and other promotional materials. Minors will not be identified on the website.
2) Registration is not complete unless registration fees and 1st month's tuition is paid in full.
3) Parent/Guardian is responsible for all tuition fees, costume fees, tights, shoes, etc.
4) The dance instructor will be notified in sufficient time of my child's absence.
5) Recital Costumes are billed to your account in October, November, and December. Payments range from $20-$35 per installment.
6) Refunds will not be given for missed classes. Make-up time can be scheduled with your dance instructor.
7) Late fees will be assessed on the 8th of every month.
8) Auto-pay enrollment is a requirement through Dance Studio Pro. for each student. Your credit card will not be charged if your fees are paid on or before the 8th of each month.
9) $35 NSF fee will be applied on all returned checks or declined credit card payment through Auto Payment.
10) I have been given a copy of Miss Michele & Co. Medical Release Form and completed all information.
11) I have been given a handout which includes Miss Michele & Co. Policies & Procedures and agree to abide by them.
By signing this document, I hereby agree to all 11 statements listed above. I understand that NO REFUNDS will be given for any and all registration fees paid towards 2023-2024 Dance Season.
