WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND IDEMNIFY AGREEMENT
In consideration of the registered dancer being permitted to participate in On The Edge Dance Studio class(es) and activities and use of facility at On The Edge Dance Studio,LLC. I further agree indemnify and hold harmless On The Edge Dance Studio, LLC, owners Kim Borgaro and Robert Gilliam, staff and teachers from any and all claims which are brought by, or on behalf of the student which are in any way connected with such use or participation in dance classes at On The Edge Dance Studio, LLC. I agree that I, my heirs, assignees, and legal representatives will not make any claim again, sue or attach the property of On The Edge Dance Studio, LLC, owners Kim Borgaro and Robert Gilliam, staff and teachers or other participants for injury or damage resulting from my participation in dance class(es) or other activities.

I release On The Edge Dance Studio, LLC, owners Kim Borgaro and Robert Gilliam, staff and teachers from any and all actions, causes of action, lawsuites, claims, or demands that I, my heirs, assignees and legal representatives now have or hereafter may have for any and all injury, illness, loss of or damage to property associated with my participation in dance class(es).

We are aware that participation in dance class(es) may be hazardous and acknowledge that a certain amount of physical health, strength and flexibility will be required. I represent that the student named above possess the level of health, strength and flexibility necessary to participate in this activity. By voluntarily participating in this activity I acknowledge the risks of injury. I will assume all responsibility and liability for any and all injuries that may be sustained due to participation in this activity, I understand that I am responsible for any medical expenses and that the absence of health insurance does not make On The Edge Dance Studio, LLC responsible for medical expenses. This authority includes the power to authorize any and all treatment deemed necessary under the circumstances by a licensed physician.

I/we also acknowledge On The Edge Dance Studio, LLC reserves the rights to use all videos and photographs that includes dancer listed above for any promotional and marketing use (print, video, online and social media).

This power is in essence a power of attorney and shall remain in effect for the culmination in participation at On The Edge Dance Studio, LLC.
