Registration and Term Policies

Thank you for choosing us as your dance education provider. We are excited to welcome you to CAD!


As parent or legal guardian for THE DANCER(S) ASSOCIATED TO THIS ACCOUNT, I hereby give permission for my child(ren) to participate in dance classes, events, ans activities at Carrollton Academy of Dance at its usual place of business, and any and all other locations provided by Carrollton Academy of Dance including but not limited to venues for shows, community performances, competitions, conventions, workshops, in route to venues, etc. I understand that dance is a physically demanding activity resulting in risk of injury to my child(ren) and/or myself if age 18 or older.

On my own behalf as well as on the behalf of my child(ren), I indemnify and hold harmless, and weigh all claims against Carrollton Academy of Dance from and against any and all loss, cost, charge, claim, liability, damage, medical expense, long or short term care, claims for emotional distress, suit or demand, known or unknown, apparent or not apparent, present and future, arising directly or indirectly from any personal injury or property loss which may occur to myself or my child(ren) on the premises of Carrollton Academy of Dance, or any other location provided by Carrollton Academy of Dance, and or under instruction, under supervision, or control of Carrollton Academy of Dance.
As the legal parent or guardian, I release and hold harmless Carrollton Academy of Dance, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Carrollton Academy of Dance, its owners and operators or in route to or from any of said premises. The undersigned gives permission to Carrollton Academy of Dance, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.

If, at any time, I or my dancer chooses to withdraw from classes, I understand that it is REQUIRED that I sign a withdrawal form. By signing the withdrawal form I will confirm withdrawal from all classes. If no form is signed, I understand that I am liable for ANY AND ALL charges that are posted to my account. This includes but is not limited too auto drafted monthly tuition, costume fees, and performance fees. I understand there are NO REFUNDS or account credits for any reason. I UNDERSTAND THAT A WITHDRAWAL FEE OF 1/2 MY MONTHLY TUITION, BUT NO LESS THAN $55 WILL BE CHARGED IN INSTANCES WHERE MY DANCER DOES NOT COMPLETE THE FULL DANCE TERM (FALL & SPRING, 10.5 MONTHS) FROM THE TIME OF REGISTRATION THROUGH THE FIRST WEEK OF JUNE.

Once registered, I will ensure that I log in to my portal through the website and become familiar with any and all information posted through the portal. I understand that no late fees will be waved for any reason during studio closings as payment can be made online through my parent portal.

TUITION- three payment options- One time payment for FULL TERM OR automatic draft each month using the debit or credit card provided.
There is a $35.00 returned check charge for any checks returned by the bank. NSF recovery fee is a total of the $35 penalty plus the original transaction fee. This payment must be cash only.

I have read and understand ALL Policies: Fees and Rates provided to me at Open House OR online at I am aware of all financial obligations associated with the 2018-2019 Term and understand the policies associated. These policies include being held responsible for any and all tuition fees, costume fees, and performance fees incurred during the term. I am aware and understand that costume fees and performance fees are optional HOWEVER, it is the parent's responsibility to provide a completed Opt Out Form by the designated date in order to refrain from being charged these fees. I am aware and understand that tuition fees will be auto drafted the 1st of each month August - June and a Withdrawal Form along with a Withdrawal Fee of no less than $55 are required to stop monthly auto payments before June.

I understand that CAD maintains the right to change or cancel the class schedule at any time. This may include, but is not limited too day, time, and/or teacher.

I understand that my dancer is required to be within dress code and have read and understand the dress code available at I understand that CAD staff/instructors may dismiss my dancer from class if they are not dressed according to dress code.

I understand that it is MY (parent, account holder, dancer, etc.) to stay current and up to date on all information pertaining to the studio. I understand that Newsletters will be emailed periodically and that to receive the Newsletters I must sign up through the website homepage.
