Please initial the below statements
________ I, the parent of the above child, hereby give my approval for her/his participation in activities during the current season. I assume all risks and hazards incidental to the activities and transportation to and from the activities. In case of injury to my child, I hereby waive all claims against Dance 4 Life Academy, the owner, instructors, and employees. I release from responsibility any person transporting my child to the doctor, or hospital in case of injury. I understand that my daughter/son has to be covered on his/her own insurance or mine to be able to dance at Dance 4 Life Academy. My child is covered by__________________________.
_________ I agree that all photos taken during the season are for promotional use.
_________ I have read the dance contract and agree to abide by it throughout August 2021 until July 30th 2022.
_________I understand if in anytime I need to break the contract, I will have to pay a $450 break the contract fee to cover choreographer fees, additional practices, competition fees and or other expenses.
_________I understand that if my child cannot attend a competition, nationals, or any other mandatory performances, that I the parent will be responsible for paying all fee's associated with the event.
_________I understand that my behavior and my child's behavior could get us kicked off the team and may risk our chances of being able to dance at the studio or compete next season.
________I understand that I the parent must respect D4L at all events in and outside the studio, as well as the staff, dancers and other team parents.
_______I understand that my dancer must attend all 2 MANDATORY competitions, March and April along with the MANDATORY NATIONALS and MANDATORY PERFORMANCES.
_______ I understand that there is a MANDATORY bootcamp prior to seasoning starting and that my competitive dancer must attend if accepted on to the team.
_______I understand that all costume fees must be paid in full by October 1st.
