Dance Studio Health and Medical Condition Policy Waiver
As a dance studio, we prioritize the well-being and safety of all our students. We understand that some students may have specific health conditions or medical considerations that require special attention. To ensure the best care and appropriate accommodations, we kindly request parents or guardians to complete the following Health and Medical Condition Policy Waiver for their child:
Health Information:
Please provide accurate and up-to-date information about your child's health conditions, including allergies, chronic illnesses, physical limitations, and any medications they are currently taking. This information will help us create a safe and inclusive environment for your child during dance classes and activities.
Medical Emergency Plan:
In the event of a medical emergency, we request parents or guardians to provide details of emergency contacts, primary healthcare providers, and any specific instructions or precautions that need to be followed. This information will be treated with the utmost confidentiality and will only be shared with authorized staff members who require it for the well-being of your child.
Medical Treatment Authorization:
By signing this waiver, parents or guardians authorize the dance studio staff to seek emergency medical treatment for their child if necessary. This includes the administration of first aid, calling emergency services, and transporting the child to a medical facility as deemed appropriate by the staff. We will make every effort to contact parents or guardians in such situations.
Assumption of Risk:
Parents or guardians acknowledge and understand that dance activities involve a certain level of physical exertion and movement, which may carry inherent risks. By signing this waiver, parents or guardians accept these risks and release the dance studio and its staff from any liability for injuries or accidents that may occur during classes or studio-related activities, except in cases of negligence or willful misconduct.
Parental Responsibility:
Parents or guardians are responsible for notifying the dance studio of any changes in their child's health condition, medications, or medical instructions. It is essential to update this information promptly to ensure the continued well-being and safety of your child while participating in our dance programs.
