CREDIT CARD PAYMENT AUTHORIZATION FORM

I hereby authorize Jan Martin Dance Studio to charge my Visa, MC, Discover or American Express credit card through Authorize.net WITHOUT a signature at Point of Sale for any and all tuition and/or merchandise purchased from JMDS. A receipt of payment and merchandise received will be emailed to me for each and every payment that appears on my statement.
