Consent form/Waiver/Studio Policies

Every student/parent will fill out a Consent Form and sign a Hold Harmless and Indemnity Agreement and Release Form.

Tuition and Payments
1. A $20 non-refundable, membership/music fee is due at the time of registration. Sometimes we run specials for this! Parents pay a membership fee each year when registering for classes.

2. Tuition is due the first of the month regardless of attendance that week. A late fee of $20 will be applied if tuition is not paid by the 10th of every month (summer tuition is due the first day of summer camps). If a student registers mid-month, they will be allowed extra classes to make up for classes missed or an adjustment will be made on your portal for a credit. Our registration is a membership, meaning tuition is due regardless of attendance. Parents are responsible for getting make up classes scheduled for classes missed. See our receptionist or email for the schedule. To dis-enroll from Pulse, parents must submit an email to pulseperformers@gmail.com if dis-enrollment is not completed properly you will still be charged for monthly tuition. Once a student dis-enrolls after the 5th of the month the full tuition amount is still due plus late fees if it hasn't been paid on time. Students can either finish their month of classes or forfeit the classes. It is your responsibility to follow the dis-enrollment process. We do charge a $50.00 dis-enrollment fee if students do not complete the session they are currently enrolled for once you enter recital preparations. Any fees paid included costume fees are forfeited if students disenroll prior to the recital. This fee is to compensate our staff for choreography and administrative fees due to early dismissal. Fundraiser credits are to be used and will not carry over if a student dis enrolls.

3. Seasons at Pulse are as follows
Season 1: August - December
Season 2: January - May
Summer Camps - June & July

4. NO REFUNDS are provided for any fees paid to Pulse (tuition, costumes, tickets, etc).

5. Cash/Debit/Credit is the only currency acceptable for payments to studio.

6. Parents are responsible for paying all balances (this may include payment for uniforms, competition entry fees, studio fees, etc.) before a student will be able to practice or preform.

Studio Rules
1. Students are expected to arrive to class 10-15 mins early with appropriate hair, shoes, and uniform for their class. Any student not prepared may be asked to sit out and watch. Any student arriving late needs to join into class quickly and quietly. Students may not leave the classroom without teacher permission, this is for the safety of the students. Parents are not permitted to enter the studios unless given permission by a staff member of Pulse.

2. Students are expected to behave in class with respect to their teachers and the other students. Any student acting in an inappropriate manner will be pulled aside and given a warning. If the behavior continues the child may be asked to sit out and a parent will be contacted. If behavior continues we may take action to remove the student from our program.

3. No excessive talking is permitted during class. We must stay on task to complete the lesson plan for the day.

4. If a class is missed it is the parents responsibility to ask for a make up class time.

5. If a students needs to be removed early the teacher needs to be notified prior to class.

6. Parents must be on time picking their child up from class. If a student is not picked up by 15 mins after their class a $10 fee will be added to your account.

7. In case of inclement weather, cancellations are posted on our Facebook page and announcement board. We also send you an email and or text message.

8. Email is our number #1 form of contact. All parents will receive information via email. It is parents' responsibility to check their email and keep up to date on their accounts. Emails sometimes get sent through spam, be sure to check that folder in your email! All emails have an option to "Star" the email, or make it a priority. We suggest doing this so you get information as soon as we send it.

9. Students are not allowed to have any food, drink, or gum in the studio. From time to time we do have holiday parties in the studio. We always check our portal for allergies. Please make sure these are always updated so we can provide appropriate snacks!

10. Students are allowed to bring food and drink to keep in the locker room to access during breaks.

Emergency Contacts
1. It is the responsibility of the parent/dancer to ensure the emergency contacts are up to date in the parent portal. In the event of an emergency you will be contacted through these avenues. If a parent/contact does not answer, a Pulse staff member will contact Miss Kayla (owner) for appropriate action. At that time action may be taken for a student to have medical attention. We are not responsible for any medical fees.

Photo/Videography
1.Any photos or videos taken by a member of the studio may be used strictly for promotional purposes of the studio.

Release and waiver of liability/Assumption of risk agreement.
1. “Events” means any and all activities sponsored or conducted by Pulse Dance and Performance Center LLC and its' staff, including but not limited to dances, games and any other associated activities.

I, FOR MYSELF AND CHILDREN, HEREBY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE Pulse Dance and Performance Center LLC, its officers, employees, volunteers, sponsors and other participants, herein referred to as “releasee” from all liability to the undersigned, his/her personal representatives, assigns, heirs and next of kin for any and all damages, and any claim or demands including negligence of the releasee while the undersigned and/or my children are competing, participating, observing, working for any purpose or otherwise participating in any event sponsored by Pulse Dance and Performance Center LLC.

I, FOR MYSELF AND CHILDREN HEREBY ASSUME FULL RESPONSIBILITY FOR THE RISK OF BODILY INJURY, DEATH OR PROPERTY DAMAGE due to the negligence of releasee or otherwise while competing, participating, observing or working for any purpose or otherwise participating in the Events.

I HAVE READ THIS RELEASE AND VOLUNTARILY AGREE TO ITS TERMS

Acknowledgment/Consent
I give consent for my child to participate in classes conducted under the supervision of Kayla Ellenwood or her employees, volunteers or sponsors. I have reviewed the tuition payment requirements, Emergency Contacts requirements and agree to the assumption of risk and waiver policy. I agree to abide by the policies detailed in these documents and attest to such in checking I accept.

By digitally agreeing to our policies through DanceStudio-Pro, you agree to the decree of this being a law abiding contract to Pulse Dance and Performance Center LLC. Families have the ability to withdrawal from our program at any time. Refer back to this official contract to follow steps for this process should you choose to.
