Media Consent Form

I agree that Creative Dance Industries Pty Ltd have the right to use my child's image in promotional, advertising material and to photograph, film, record, edit, broadcast and reproduce such material for Gabba Dance / ASDP and related entities without any geographical media or time restrictions whatsoever.

It is also agreed that no payment is agreed as full and final consideration for my participation and the use of my image as set out in this agreement and that no further claim, suit or demand can be made against Creative Dance Industries Pty Ltd.

All materials produced pursuant to this agreement and all rights therein including copyright and other intellectual property shall be the sole and absolute property of Creative Dance Industries Pty Ltd or its assignees or nominees.
