RELEASE AND WAIVER OF LIABILITY FOR PARTICIPANTS OF WEST CHESTER DANCE WORKS/DIANE MATTHEWS SCHOOL OF DANCE ARTS
I understand that participation at West Chester Dance Works has risks due to the physical demands that are placed on an individual during physical activity and recreation, and that during physical activity there is a potential risk of physical injury. I agree that I am solely responsible for myself or my child’s participation and for my or their physical and emotional wellbeing. I understand that the program activities are voluntary. I am choosing for myself or my child to participate in each dance activity, rehearsal, or performance to whatever degree possible-with all physical, emotional, and medical conditions considered.
I affirm that my and/or my child’s health is good, and is not under a physician’s care for any undisclosed condition that bears upon their fitness to participate in dance, physical education, and recreation. I willingly and knowingly assume risk, for my child, myself, my heirs, family members, executors, and administrators and assume all risk of physical injury and emotional upset which may occur during any aspect of the program; and hereby agree to hold West Chester Dance Works, its employees, independent contractors, instructors, facilitators and volunteers harmless for any liability arising out of my or my child’s participation in the program.
This release does not, however, apply to any physical injury or emotional harm caused by negligence or willful misconduct of West Chester Dance Works.
AUTHORIZATION TO UTILIZE AUDIOVISUAL PRODUCTS
West Chester Dance Works has frequent occasions to illustrate and explain our program and activities for fundraising, enhancing community awareness, news releases, brochures, reports, etc. Toward these efforts, it is most beneficial to use photographs of our dancers. However, we would never intentionally offend our dancers by doing so without their understanding and consent. If you feel comfortable allowing you or your child to be photographed or videotaped, we would appreciate your consent.
