Liability and Consent Waiver

I agree to hold harmless Destination Performing Arts Center from liability from any injury occurring on the premises of Destination Performing Arts Center or any Destination Performing Arts Center event. I give consent for Destination Performing Arts Center staff to render appropriate judgment should my child have an accident or need medical attention and I, nor the emergency contact, are able to be reached. I also understand and agree to alert DPAC in writing immediatlely of any injury my child may have acquired on the premises.
