Studio Waiver

I understand payment options and the late fee policy. I understand and accept all tuition policies.

I hereby release Partners Dance Studio LLC and Genese Brisky, Director, from any and all liability and/or obligations which would result from accident or injury which may occur in or on the property of Partners Dance Studio LLC, or while participating in dance classes scheduled by Partners Dance Studio LLC and/or Genese Brisky.

I hereby give to Genese Brisky, or anyone she my designate, authorization to contact any medical and/or emergency assistance she may deem necessary.

I understand that Partners Dance Studio LLC and/or Genese Brisky will not be responsible for any medical payments or fees charged as a result of any accident or injury or the emergency treatment thereof.

I hereby release photos and video footage for Partners Dance Studio LLC publicity.
