*By signing below, adult of guardian assumes full financial responsibility for all tuition and fees. If for any reason dancer decides to no longer be a student at Studio Geaux, he or she must present instructor with a written notice and will be responsible for payment of any classes attended. Additionally, said person agrees to read and abide by all studio rules and payment policies. **There are no refunds for registration fees, tuition, costumes, apparel, private lessons, or recital.**
