Elite Formation Studio of Dance Waivers/Policies

Auto-Pay:
I authorize Elite Formation Studio of Dance to charge the credit card that's in the system for agreed upon dance tuition. I understand that my information will be saved to file for future transactions on my account.
Withdrawal/ Cancellation Policy:
I understand the Elite Formation Studio of Dance requires a written(email) 30-day notice for withdrawal from their dance program. Please email: eliteformationstaff@gmail.com to withdraw your student. Once your email has been confirmed your 30-day period will begin. If your 30-day notice has been confirmed after the first of the month, your card will be charged for an additional 30 days, in which your child is still welcome to take classes until the 30-day period has been completed. After your 30-day period is complete your account will no longer be charged. Your dance studio account will be made inactive, and your credit card information will be removed from our system. If you choose to re-enroll your child, I understand I will have to pay the registration fee to re-enroll my child in class, even if it's in the same dance season.
Non-Refundable Fees:
All payments made to Elite Formation Studio of Dance are non-refundable after your first 30 days. You have 30 days to cancel out of our dance program if our services do not match your dance needs. If you are within your 30-day grace period, your monies will be refunded within 30 days after your cancellation. Registration fees are non-refundable.
Costume Policy:
I, the Parent, understand that I must coordinate with The Studio to provide the appropriate measurements for my child's costume(s). If the measurements change, I, the Parent, understand that it is my responsibility to purchase the appropriate costume(s) that fit, if The Studio is unable to return the original costume(s) in its original state. (*note that the additional purchase of the costume(s) includes shipping charges to return/resend).
Privacy:
When you register for classes at Elite Formation Studio of Dance, we collect contact information such as guardian names, addresses, phone numbers, emergency contacts, e-mail address and student information such as student names, birthdays, medical conditions. Some of the information is required on the forms. This information is needed to manage the studio business and will not be used or sold for any other purpose, except as described below. Some of the information is optional, optional information is requested to help us better understand our customers and is used only as needed for studio business.
Social Media:
I understand that Elite Formation Studio of Dance may take pictures and or video during classes or any event for social media and advertising purposes. All pictures and videos will be appropriate in nature. These photos and or videos may be used for websites, printed media, and all social media platforms. If you are uncomfortable with us using your child's photo or a video with your child in it, please email the staff at eliteformationstaff@gmail.com to ensure your request is noted. If you do not send us an email, we cannot ensure your request will be adhered to. We must be notified via email to comply with this request.
Tuition Policy/ Past Due Payments:
Tuition payments are due on the 1st of the month for our services. Tuition fees are based on a 12-month installment plan, and the same fee is charged each month, regardless of how many classes were held in that month. Our Tuition AutoPay system is MANDATORY and it EASY to set up and manage your monthly tuition payments. They will be taken care of automatically and you never need to worry about late fees again!
