Liability Waiver

This agreement releases MIMPA from all liability relating to injuries that may occur during activity on premises, in class or backstage at the theatre.
By registering my child, I agree to hold MIMPA entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence. I also acknowledge the risks involved in performing arts and dance classes. These include but are not limited to strains/sprains, trips and falls, collisions, set or prop related injuries and injuries incurred while in rehearsal or performance.
I swear that I am allowing my child to participate voluntarily, and that all risks have been made clear to me. Additionally, my child does not have any conditions that will increase their likelihood of experiencing injuries while engaging in this activity. If my child does, I will inform staff immediately.
By signing below I forfeit all rights to bring a suit against MIMPA for any reason. In return, my child will receive performing arts, music and dance classes.
I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.
I understand that MIMPA is following all protocols required by the Ontario Government and the South Simcoe Health Unit. As at any other place, MIMPA cannot guarantee that my child will not be exposed to viruses (at MIMPA or elsewhere), or if exposed, that my child will not contract the disease. By sending my child to MIMPA, I agree to waive any claims of liability against MIMPA and to hold MIMPA harmless if my child contracts an illness despite MIMPA's best efforts to prevent it. I promise to not send my child to class/performances/rehearsals if they are sick or contagious with any communicable disease. This action will help ensure the safety of MIMPA's students and staff.
