Coronavirus (Covid -19) Parent/Guardian Agreement

To protect Kelly Richards School of Dance students and staff, I agree to keep my child home if he/she has:
1. Tested positive or have been deemed presumptively positive with Coronavirus.
2. Has been informed that you may have been exposed to Coronavirus.
3. Experienced any of the following systems commonly associated with Coronavirus including:
*Fever*(a temperature of 100.4 or more) *Cough* *Congestion or runny nose* *Nausea or vomiting*
*Diarrhea* *Sore Throat* *Chills* Muscle pain or aches* *Headache* *Fatigue*
* New loss of taste or smell*
If my child has any of these signs of COVID-19 or has been diagnosed with COVID-19, I will not send him/her back to dance until:
*My child tested negative for COVID-19 and is otherwise well enough to go back to activities
OR
*A healthcare provider has seen my child and documented a reason for the symptoms other than COVID-19
OR
*All the following are true: 1.) it has been at least 10 days since the start of symptoms AND 2.) the child is fever free, off anti-fever medications for 48 hours AND 3.) symptoms are getting better.
If someone in my household is exhibiting symptoms of COVID-19, is diagnosed with COVID-19 or my child is exposed to COVID 19, I will keep him/her home for 14 days.
