Agreement for Participation
I understand that dance classes may include, without limitation, dancing with props, stretching, across the floor combinations, dance routines in the center, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. Head To Toe Dance Studio is not responsible for personal property.
I understand tuition is due by the 10th of each month enrolled in class. --
Late Fee Charge a $15.00 fee 1 day after the due date. This is a one-time fee.
Tuition late fees will be added to only one student per family. I understand to unenroll I must log in my studio portal and drop the class. I understand that I will pay tuition every month enrolled at Head To Toe Dance Studio. I understand if I do not pay legal action will be takin.
I agree to be responsible for reading studio correspondence and respecting deadlines.
