DO AGREE TO USE OF CHILD’S IMAGE
I , parent of Participant, DO hereby grant permission and consent to Thrive Performing Arts Inc. (“Thrive”) and any third party that Thrive hires, contracts or utilizes, to obtain or otherwise take images of my child and to use said images of my child in promotional materials for Thrive. Such use includes, but is not limited to displays, distribution, publication, transmission, or other use of photographs, images, and/or video of my child for use in materials that include, but may not be limited to, printed materials (such as brochures, newsletters, posters, and other print advertising), digital materials (such as Thrive’s website, electronic newsletters, blog, and emails), and social media (such as the Thrive’s Facebook page, Twitter feed, or other social media platforms)(hereinafter referred to as “Material”). I further understand that Thrive has exclusive rights to any Material it produces and I agree, that I or my child will not receive or seek any payment or compensation from Thrive, including royalties or any other interest, for signing this release or as a result of any publication or distribution of the Material by Thrive or any third party Thrive hires, contracts or utilizes. I on behalf of my child, hereby waive any right to enforce a payment for signing this release and for any publication or distribution of the Material.
Furthermore, in addition to agreeing to permitting Thrive to take videos or photos of my child, I:
Agree to permit Thrive to use my child’s name in conjunction with any Material for the purposes stated above.
