Registration waiver

Non-refundable registration fee is $30 per student or $50 per family. Tuition is due on the 1st of each month & no later than the 10th. Payments received after this date will be charged a $5 late fee. Returned check fee is $25 and a check redeposit fee is $10. These fees are what our bank charges us. If your account is more than 2 mo this past due, your dancer will not be allowed to attend class until the account is current. This contract is for the school year August through April. Tuition for the half month of May will be charged in August with your half month of August tuition to create a whole month’s tuition and to eliminate the payment in May. Costume fees for our spring recital are also non-refundable and are due in October. Costume orders will be placed by Dec 31st and no late orders will be placed. Costume fees are subject to change and begin at $100 per student. Ages 2-5 years receive a new costume to keep and all other classes will rent one costume per subject unless otherwise stated. By enrolling in class(es) at The Dance Company, I agree to allow photos to be taken of my child For promotional purposes to be used in print, on social media & on The Dance Company website. No names will be listed with photos. I further agree to provide written or verbal 30 day notice to disenroll my child from classes to avoid being charged tuition for that month. By signing this form, I agree that I am the responsible party for payment on the account and I understand and accept the risks involved with physical activities my child will/has engaged in while at The Dance Company ( such as but not limited to dance and tumble) and during Events outside The Dance Company that we may participate in as a group. I agree to hold harmless The Dance Company, Lani Milling & its affiliates for any injury received while participating in any activities. By signing this form, I agree to these Terms and and state that I have read & understand them as they are printed. I further understand, agree that if I choose not to sign this form that I may be denied enrollment.
