CODE OF CONDUCT CONTRACT
*This includes all that is mentioned in the CCPA Handbook.
CONDUCT RULES:
BE RESPECTFUL AND AGE APPROPRIATE. We expect our dancers to uphold age appropriate and respectful behavior at all times. If your actions are not deemed appropriate and respectful and do not coincide with this organization's guidelines you will be denied participation at CCPA.
PRACTICE ETIQUETTE AND BEHAVIOR:
Only three verbal warnings will be given for any unruly behavior in class. This includes talking, non-participation or any behavior disruptive to the class. A dancer will be asked to leave practice in lieu of any other warning.
No cell phones are allowed during class.
WITHIN AND AT CCPA, PERFORMANCE VENUES, CONVENTION VENUES, WORKSHOP VENUES there will be:
NO ILLICIT DRUG USE - Any person or persons partaking in the use, selling, sharing, carrying of any kind will be expelled from the studio. This includes any alcohol consumption and smoking cigarettes, e-cigs, vapes, and hookah pens.
NO ROMANTIC RELATIONSHIPS - RELATIONSHIPS MUST BE STRICTLY PLATONIC
no talking, no texting, no sexting, no group dates, no dating
no kissing, no hugging, no holding, no cuddling
keep all body parts to yourself
THIS INCLUDES - all teachers, students, clients, future students and past students
MEDIA Waiver:
I hereby consent to allow the use of voice, video, image or likeness in photographs and/or video for my child(ren) by California Conservatory of Performing Arts and MJM Dance Company.
The permission for use of any of the media above is allowed for as follows
Newsletters
Business Flyers/brochures/postcards
Facebook
Company Website
Video & Photo by a third party (such as filming for a television commercial and competitions)
Outgoing messages on answering machines and/or voice mail
I understand this Waiver is in effect until I provide, in writing, a cease and desist order. I also agree to forego any right or entitlement to said media.
No videotaping or photography unless otherwise stated by the director. Please know that we encourage you to take photos and document your experience with CCPA.
Assumption of Risk:
I, the minor's parent or legal guardian, understand the nature of the activities my child will be involved in at California Conservatory of Performing Arts, and the minor's experience and capabilities, and believe the minor to be qualified, in good health, and in proper physical condition to participate in such activities.
PROBLEMS/ISSUES:
If you do have a problem we need to know in order to help resolve the situation. Problems that occur are to be solved by the student(s) and the director only. Parents will be allowed to assist with studio problems only when asked to do so by the director. The student must report any problems or concerns to the director immediately. This includes injuries, illness, issues with fellow teammates and/or parents, etc.
UNIFORMS/PRACTICE ATTIRE:
Dancewear is required at ALL practices. A leotard/shorts/crop top (tights optional) and proper dance shoes. No jeans. If proper attire is not worn you will be sat out of practice! Clothes must be kept clean and in good repair.
FAMILY/FRIENDS:
We have a CLOSED STUDIO DOOR POLICY at ALL practices. It is our experience that students are better focused without the distraction of parents, siblings, and friends. Family and friends can watch in the room next to reception. Family and friends are not allowed on the studio floor during practices.
LATE POLICY
Any child more than 10 minutes late will not be admitted into class - this will count as an absence
If you know your child will be more than 10 minutes late then CCPA must be informed 24 hours in advance, not the same day as the class they will be late to.
It is required that each dancer and a parent/guardian be present for all parent meetings.
If a member knows in advance that they will be late or will be missing a practice/performance, they are to advise the director immediately. Team members must be present at every mandatory practice for the entire practice time in order to be eligible to participate in upcoming events, performances, competitions, and conventions.
If you miss a practice you must make up the material you missed BEFORE the next practice. IT IS YOUR RESPONSIBILITY TO CATCH UP!
Release of Liability:
I hereby release, discharge, covenant not to sue, and agree to indemnify and save and hold harmless each of the releasee's from all liability claims, demands, losses, or damages on the minor's account caused or alleged to be caused, in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations. I further agree that if, despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the releasees named above, I will indemnify, save, and hold harmless each of the releasees from any litigation expenses, attorney fees, loss liability, damage, or any cost that may occur as the result of any such claim.
Medical Emergencies:
In any event of an accident or emergency I hereby authorize my child to be transported to a hospital for medical treatment and I hold California Conservatory of Performing Arts and it's representatives harmless in the execution of such. Additionally, I hereby agree to individually provide for all medical expenses which may be incurred by me or my child(ren) as a result of any injury sustained while participating at or for California Conservatory of Performing Arts.
Studio Policies:
Please review and agree to our studio policies:
Parking lot- DO NOT EVER LET YOUR CHILD PLAY IN THE PARKING LOT. We often see parents chatting in the lobby after class while their child walks out the door without them. We also see parents chatting outside the studio while their child plays between cars or in the neighboring business parking. This is absolutely against studio policy. Children are not to be left outside without supervision or leave the building without an adult.
Where to park- We do not have designated parking spots; Please be courteous to neighboring businesses and do not park in front of their garages (in the back) for more than 5 minutes during their operating hours.
Send your student(s) to the studio prepared for class- Have the student(s) dressed properly (please see website for specifics) and hair pulled back.
Viewing window- The viewing window gives you an opportunity to watch class every so often and see the student's progress. Please feel free to watch class. If you notice students paying more attention to you than the instructor, please move away from the window. You might be a distraction to other students in the class, or possibly the teacher. Do not allow siblings to stand in lobby chairs while watching through the viewing window. Please make sure siblings are not distracting class.
The lobby- Families are always welcome in the lobby but please keep in mind that lobby space is limited and it gets really hot in the waiting area. If you utilize the lobby, please throw trash away and straighten toys before leaving. Please replace chairs if you move them.
Before and after class- Most classes start as a class ends. Please gather your child and be prepared to leave the building after class is over. Please be courteous to the class that comes after your class is over.
Communication- The best way to communicate with teachers is by emailing through email or calling the studio. Messages may not be returned the same day. Teachers cannot accept calls during class time. Text message is best.
PAYMENT POLICIES
1. Tuition is based on a monthly price
2. Monthly Price - Fees are based on an average of four classes per month. No additional charge is applied when a fifth lesson occurs in a month. Also, there is no reduction/prorate when fewer than four classes are held; due to a holiday, vacation, absence, conventions, or performance.
3. Makeup - A Makeup lesson is a selected class for a student to join that is similar to the missed scheduled, non-company class or private lesson. The Makeup lesson will only be valid for 30 days, starting the day after the absence and the class the student chooses to join must have the same time value as the missed class. Please make arrangements for a makeup class with the studio director. There are no makeup lessons for Unlimited Students or for Maxed Out Students.
4. Unlimited Students - Are students who take/pay for 8 or more hours a week.
5. Maxed Out Students - Are students who take/pay for all classes within their age range.
6. Studio Age Ranges:
A. 3-6yrs
B. 7-11yrs
C. 12-15yrs
D. 16-17yrs
E. 18+yrs
7. Payment of Tuition is due the 22nd of every month prior to enrolled month, in order for your child to participate in their class(es) for the upcoming month. Students may not participate in class until the tuition is paid in full. If payment is not received by the 27th of the prior month, there will be a late fee of $20.

9. Returned check fee: $25.00
10. ALL TUITION FEES, COMPETITION FEES, PERFORMANCE FEES, RECITAL FEES and COSTUME FEES ARE NON-REFUNDABLE
REFUNDS: Please be advised that any deposits/money put toward fees, uniforms, warm-ups, t-shirts, registration fees, tuition, or any other item is NON-REFUNDABLE. This is regardless of whether a Dance Team member is suspended, injured, or removed from the team or not! -
I, the parent/guardian of the above named, have read the Code of Conduct and understand all of the requirements. I fully comply with all rules, regulations, payment of fees and tuition within the CCPA Handbook. I also realize this is a team effort and I will be called upon to help in some way throughout the year with things that are within my capacity to help. I will make every effort to do my share of assisting on behalf of my dancer. I understand I am required to be present for parent meetings or have someone present on my behalf.
By checking below I understand and agree to what is stated above. I understand and agree to pay on time or additional costs will occur. I understand and agree that all fees are non-refundable. I understand and agree that by not paying I am forfeiting my or my dependent's spot(s) in class.
