NOTICES:

PLEASE READ THE FOLLOWING IN ITS ENTIRETY:
Payments, Late Fees: Tuition is charged on an annual basis. Annual tuition may be paid all at once, however many chose to pay in 10 installments, or monthly. We do not charge monthly tuition, rather our annual tuition is conveniently broken into payments to assist our families. If you choose to make monthly payments, these are due the first class of each month. Any payments made after the 15th of each month will automatically incur a $10 late fee. Payments can be made in the Studio, mailed, online by credit card, or by enrolling in Auto-Pay. If you choose to enroll in Auto-Pay you will be required to fill out a form online that automatically debits your account each month.

Withdrawal and Refunds: One-month notice is required to discontinue any classes. Withdrawal must be done in writing or by email, and will not be accepted over the phone. No refunds for discounts for missed classes. Make-up classes are available upon request.
Adult or Parent Responsibility: It is the responsibility of the adult or parent to be aware of all activities, such as class rescheduling, Parents Watch Week, and Studio closures. The school will post all such notices on the Announcement bulletin boards, the TV in the lobby, and on front door. It is the adult or parentâ€™s responsibility to regularly check these places as well as the email provided to ensure they are informed. It is also the responsibility of the adults or parents to inform the school of any email, address or telephone number charges.

Costume Fees: Costumes are $90 per costume, per class. Costume deposit fees for the year-end recital will be charged October 15th. No costume fees will be refunded after this date even if a student withdraws from the school. Costumes are not required for the Musical Theatre portion in the recital.

Photo Release: The Studio is herby granted permission to take photographs of the students to use in brochures, websites, posters, advertisements, and other promotional materials the studio creates. Permission is also herby granted for the school to copyright such photographs in its name. *If you wish to Opt-out of the Photo Release, please email studiodschoolofdance@gmail.com and state that you wish to Opt-out of releasing your child's photo.

Injuries: As the legal parent or guardian, I release and hold harmless Studio D School of Dance and its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Studio D School of Dance owners and operators or in route to or from any of said premises.
