DFLP LLC Operation Policies
As legal parent or guardian of ___________________, I acknowledge receiving and reviewing the policies set forth by DFLP LLC. I understand it is my responsibility to uphold these policies set by DFLP LLC.

Release of Liability
As the legal parent or guardian, I release and hold harmless DFLP LLC, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or any guest accompanying the participant, while in or upon the premises or any premises under the control and supervision of DFLP LLC, its owners and operators or in route to or from any said premises.
Parents, legal guardians of minor students and adult students waive the right to any legal action for any injury sustained on school property resulting from normal dance activity conducted by the students before, during or after class time. The undersigned gives permission to DFLP LLC, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I agree to provide the doctor/physician name and number to be called in the case of an emergency.

Photo/Media Release
The school is hereby granted permission to take photographs of students to use in brochures, web sites, posters, advertisements, and other promotional materials the school creates. Permission is also hereby granted for the school to copyright such photographs in its name.

DFLP LLC Specific Policies
Dancers must give DFLP LLC a minimum of a 2 week notice if they will be withdrawing from class for any other reason than immediate health issues.
Our classes will NOT be prorated should you miss class OR if you sign up in the middle of a month that is already in progress.
