Liability Waiver
I understand that dance classes may include, with out limitation, dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all of the activities of the dance class may involve some degree of risk or strain or bodily injury. The Maryland Academy of Dance is not responsible for any risk or bodily injury associated with participation in dance classes.
The Maryland Academy of Dance is not responsible for the loss of personal property.

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization (WHO). COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

The Maryland Academy of Dance (MAD) has put in place preventative measures to reduce the spread of COVID-19; however, MAD cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending MAD could increase your risk and your child(ren)’s risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending DSOD and that such exposure or infection may result in personal injury, illness, permanent disability, and death.

I understand that the risk of becoming exposed to or infected by COVID-19 at MAD may result from the actions, omissions, or negligence of myself and others, including, but not limited to, MAD employees, volunteers, and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that

I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at MAD or participation in MAD classes. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless MAD, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of MAD, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any MAD program, class or event.

I understand that there are risks of physical injury, illness and/or death associated with, arising out of and inherent to the participation in classes and activities associated with taking dance at The Maryland Academy of Dance (MAD). In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to The Maryland Academy of Dance, LLC, dba MAD, it's officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as “The Maryland Academy of Dance, LLC

I hereby agree to release MAD and hold MAD harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant.

I am aware that this is a release of liability and an acknowledgment of my voluntary and knowing assumption of the risk of injury or death. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.

I also give MAD permission to use my child's picture in or on any form of advertisement for MAD or a MAD affiliated event. (Please notify us in writing, if you do not wish to have your child's picture in any publications). info@mdacademydance.com

The participants have my permission to participate in all MAD related activities and events. I warrant the information I have provided is complete and correct and that I do have authorization to speak and act on behalf of all participants I enroll in classes and activities at MAD. I further release MAD of all liabilities associated with my child's attendance through MAD.

Tuition is due on the 1st of every month from Sep 1, 2024 - Jun 1, 2025. All accounts will need to placed on Auto pay or paid electronically through your DSP Account.

Tuition Late Fee: $40.00
Return Check Fee: $40.00

Membership fees are ONLY prorated to new students who join after the beginning of the month.

Payments made after the due date of the 1st will incur a $40 late fee on the account. Accounts that are past due by 1 month, the student is not allowed to attend classes until the account is up to date.

MEMBERSHIP/TUITION FEES ARE NON REFUNDABLE

Throughout the year, there may be times when The Maryland Academy of Dance
staff may take photographs of students, audiotape/videotape students, or interview
students for website advertisement and/or flyers related to The Maryland Academy of Dance.
I give permission for The Maryland Academy of Dance Academy to use my child’s
photograph and/or videotaped image for the purposes mentioned above. I
understand and agree that The Maryland Academy of Dance may use photographs
or videotaped images in subsequent school years unless I revoke this authorization
by notifying the school Directors in writing.

To withdraw from the Maryland Academy of Dance, submit a notice of withdrawal by completing a google form. The fee to withdrawal is equivalent to one month of tuition/membership. This is in addition to any fees owed on the account.
