I, parent/guardian (or myself, if age 18 or older) understand that photographs, video footage, digital mediums, or audio recordings may be used by Ecclesia School of the Arts, LLC (ESA) in whatever way they deem beneficial, including promotional materials, publications, and electronic media. I consent that such photographs, films, recordings, tapes, and other media materials are the property of ESA and they shall have the right to sell, duplicate, reproduce, and make other uses of these materials as they desire free and clear of any claim, rights of compensation, or ownership whatsoever on my part. I acknowledge that participation in the activity described above involves risk to me (and parent/guardian if minor), and may result in various types of injury including, but not limited to, the following: sickness, bodily injury, death, emotional injury, personal injury, property damage and financial damage. In consideration for the opportunity to participate in the activity described above, I, (or parent/guardian) acknowledge and accept the risks of injury associated with participation in and transportation to and from the activity. I, (or parent/guardian) accept personal financial responsibility for any injury or other loss sustained during the activity or during the transportation to and from the activity, as well as for any medical treatment rendered to me, the participant, that is authorized by ESA or its agents, employees, volunteers, or any other representatives. (collectively referred to hereinafter as “Activity Sponsor”). Further, I (or parent/guardian) release and promise to indemnify, defend, and hold harmless the Activity Sponsor for any injury arising directly or indirectly out of the described activity or transportation to and from the activity, whether such injury arises out of the negligence of the Activity Sponsor, myself, or otherwise.
