Assumption of Risk
I acknowledge that participation in classes/activities at Evolution X Performing Arts is voluntary, and I understand (a) the risks and dangers that I /my dancer(s) may encounter in this activity, and (b) how to use reasonable care to avoid or minimize those risks and dangers.
Release
I knowingly accept and choose to encounter all risks associated with my/my dancer’s participation in this activity. I understand that Evolution Staff and Instructors are not liable for any injury, loss, or damage to students, guests or their personal property that may occur while in the facility.
Medical
In the event of an emergency, I understand that every effort will be made to make contact with a parent or emergency contact that I have listed. If contact cannot be made, I hereby give authorization to Evolution X to seek medical attention for me/my dancer(s). I understand that all medical expenses incurred will be my responsibility.
Right of Admission
I understand that the dance studio is a positive, family environment, and that Evolution X reserves the right of admission, and the right to expel any student, parent or guest, for inappropriate or destructive behavior.
Tuition Payments and Cancellations
All tuition and registration fees are non-refundable. Tuition is paid on the 1st of each month by cash, check, credit card or AutoPay. Any payment received after the 7th of the month will result in a $20 late fee. There is a $35 fee for each transaction returned by the bank.
If a student chooses to discontinue their classes, we require that a 2-week written notice be submitted to the front desk. There will be no refunds for tuition already paid.
Payment plans are available for certain circumstances and must be set-up with the front desk.
Private Lessons
Private lessons are paid to the front desk, in advance. If there is a need to cancel, I understand that the cancellation must be sent via email to info@evolutionxdance.com.
There is a 24 hour cancellation policy, as this slot has been reserved for you/your dancer. Paid fees will not be rolled over for lessons that are cancelled on the same day.
Dress Code
Dancers MUST have the appropriate shoes and be in appropriate dress code attire (including hair), in order to participate in class. Please visit our website for dress code details. Students that are not in proper attire may be asked to observe for the day.
Class Attendance, Tardiness and Make Up Classes
Tuition must be paid-to-date, in order to attend class. A courtesy call/email is required if your dancer will be late or absent. Dancers that arrive more than 7 minutes late to class, may be asked to observe for the day. Late arrivals prevent dancers from being properly warmed up, putting them at risk for injury. They also cause distraction to a class that is already in session.
Make-up classes may be taken within 30 days of the missed class and should be scheduled with the front desk only. Dancers will be given another class in the same or similar level to attend, to make-up their missed class.
Photography Release
I understand that by participating in classes and events at Evolution X, my child may be photographed, or video recorded from time to time. I authorize Evolution X to use these photos by posting on the studio website, social media sites and studio related advertisements. I understand that if I DO NOT want my child pictured, I must submit this in writing, to the front desk.
Recital
To participate in the annual recital, all tuition and costume fees must be paid by the appointed due date. For details, please see additional recital information, located in the studio lobby.
Class cancellations
If at any time, a class does not meet the minimum enrollment requirement, it may be combined with another class or cancelled.
Evolution X follows the Broward County School Schedule, but please be sure to see our scheduled closing dates on our website.
Tuition is not refunded or pro-rated for any classes or cancellations due to federal or religious holidays, school breaks or severe weather. In the event of a cancellation due to severe weather, we will notify you via email by 2:45pm, as well as post on our website.
Our make-up class policy applies to the above.
Facility
Please do not open the door to the dance room, for any reason, when classes are in session. Our bathroom facility is located inside the dance room; therefore, our lobby guests will not have access to it while classes are in session. We thank you for your understanding and cooperation.
Small children must always be supervised by an adult.
Parents are welcome to drop dancers off for class, but please be on time for pick-up. We understand that sometimes things happen, please give us a call if you are going to be late. Fees of $1 per minute, after the first 5 minutes, may be charged for late pick-ups.
Pets are not permitted anywhere inside the facility.
Observers are not permitted in the dance room unless approved by the instructor.
