Due to the fact that COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact and high contact surfaces, Prodigy Performing Arts has put in place preventative measures to mitigate the spread of COVID-19; however, PPA cannot guarantee that your student(s), your family members, or yourself will not become infected with COVID-19 by participating in PPA programs or while participating in host facilities. By signing this agreement, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that your student(s), your family members, and yourself may be exposed to or infected by COVID-19 by attending programs, or while inside host facilities. You agree that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of your student(s), your family members, yourself, and others, including but not limited to PPA employees, employees and owners of host facilities, fellow students, and their families. You agree to assume all risks and accept sole responsibility for illness, injury, or damage that could arise as a result from your student(s) participation. You agree to comply with any and all health and safety procedures put into place by PPA or its host facilities.

I assume financial responsibility for tuition and other associated fees to be paid in a timely manner. I agree to the policies and terms listed below.

Registration Fee:
I agree to pay an annual registration fee, which is required for school year programs that take place August- May.

Tuition:
Tuition payment is required for participation.
For students registered through Prodigy Performing Arts, monthly tuition is posted in your account on the 25th of the month with an AutoDraft ran on the 1st of each month. I agree to add a bank account or card to my account/parent portal upon registration. Tuition is calculated per the season and then divided into equal monthly payments regardless of the number of classes per month, i.e. some months may have 3 classes per month due to holiday breaks, some may have 5. Performances and any extra rehearsal time scheduled for a performance is factored into tuition and is valued at the same rate as a class.

Some private school or afterschool programs that are held at school locations may be subject to semester payments due prior to my child's first class- no Monthly Tuition AutoDraft will be run on these accounts.

Late Fee:
A Late Fee of $10 will be applied to any account not paid by the 5th of the month.

Production/ Recital Fees:
If my child is participating in a class or activity that includes any recital, showcase, or off-site performance, production and costume fees are non-refundable.

Returned Check/ NSF Fee:
There is a Returned Check Fee or Non Sufficient Funds fee of $25 for checks returned unpaid by the bank or declined card payments.

Attendance:
I understand and agree to the policy that there are no refunds or credits available for classes, workshops, or camp days not attended by student. There are no provisions for "make-up classes" for students that miss a class, regardless of reason. If, in any event, that a class has to be canceled by the instructor or director, the class will be rescheduled. If a class can not be rescheduled, than a credit will be given to accounts of students affected by the cancellation.

Registered Student Withdrawal:
To withdraw from any class, a written notice must be received 30 days in advance of the registered student's last class.

I will allow photo images and video footage of my child participating in classes, camps, programs, and performances to be used for promotional purposes without payment to child or parents for said images.

As the parent or legal guardian, I release and hold harmless Prodigy Performing Arts LLC, or the host facility, recreation center, or school owners and all staff from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss of personal items or injury that may be sustained by the participant and/or the undersigned, while in or upon the premises or while participating at off-site locations. I assume the risk of participating in any classes, camps, or programs. I hereby declare the partici­pant to be in good physical and mental health. There are no medical or physical conditions that would hamper my child’s ability to partici­pate in any exercise program or physical activity. I authorize Prodigy Performing Arts LLC staff to seek med­ical help in the event of an emergency, if I can not be reached.
