WAIVER OF LIABILITY

I______________________________________, unconditionally release and discharge Momentum Dance Project and all its directors, staff, and volunteers from any and all liability arising from, related to, or connected with any injury, illness, or damage caused by, resulting from, or sustained in the course of my child’s participation or my participation in classes (in person or online), performances, or other activities conducted by or associated with Momentum Dance Project. I attest that this waiver of liability is provided voluntarily and shall be fully binding upon my heirs, next of kin, executor, administrator, and/or personal representative.

PHOTO RELEASE
I _____________________________________, hereby grant and authorize Momentum Dance Project, LLC/ Tara Cacciatore the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of me/my child to be used in and/or for legally promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration with no limitation on duration of seasons or years but indefinitely. This authorization extends to all languages, media, formats and markets now known or hereafter devised. This authorization shall continue indefinitely, unless I otherwise revoke said authorization in writing. I understand and agree that these materials shall become the property of Momentum Dance Project, LLC/ Tara Cacciatore and will not be returned. I hereby hold harmless, and release Momentum Dance Project, LLC/ Tara Cacciatore from all liability, petitions, and causes of action which I, my heirs, representative, executors, administrators, or any other persons may make while acting on my behalf or on behalf of my estate.
I have read this release before signing below and I fully understand the contents, meaning and impact of this release.

TUITION POLICY
I understand that tuition is due on the 20th the month prior to the month of tuition (example: January tuition will be due on the 20th of December). A late fee of $25 is assessed 10 days after the due date. I also understand that to unenroll my child and to be relinquished of charges for the remaining tuition of the year, I must send an email to info@momentumdanceproject.com 30 DAYS PRIOR.

I understand that tuition for the year may be broken up into 9.5 payments or paid for in full at the beginning of the season. Monthly payments are not directly associated with the amount of classes held within that month and therefore does not provide refunds for studio closings already accounted for for the season.

I understand that no refunds will be given for any tuition, costumes, tickets, etc. paid for. Credit may be given if extenuating circumstances arise.

PRIVACY POLICY
The privacy of your personal information is very important to Momentum Dance Project. We will not sell, rent or trade your personal information to any third party. Special care will be taken to protect the privacy and security of your information. This policy explains our information practices and the way that Momentum Dance Project collects and uses your information. This Privacy Policy covers the information you provide to us offline, www.momentum-danceproject.com, as well as our online parent portal.

You can visit our website and find out about Momentum Dance Project's products and services without giving us any personally identifying information about you. When you visit our website, we do not collect any personally identifiable information without your consent. We collect and store information from you that you voluntarily choose to give us or enter on our website. When you register for classes at Momentum Dance Project, we collect contact information such as parent/guardian names, addresses, phone numbers, e-mail address and student information such as names, birthdays, grade, etc. This information is needed to manage studio business and will not be used or sold for any other purpose, except as described below.

Momentum Dance Project will access your personal information and/or share it with third parties only for the following purposes:

-as needed for studio business
