You must be over the age of 18 to register for an account.

Please fill in all information required, select which classes you would like to enroll, and click submit. For your convenience and ours, ALL accounts will be on AUTOPAY. Tuition balance will be automatically charged on the FIRST day of the month (or first day of the season). Please contact us if you cannot setup autopay for any reason and we can assist you with registration.

TUITION balances will be billed automatically using the credit card registered on file on the 1st of each month. If another payment method is preferred, balances must be paid using another method before the 1st or else the credit card on file will be charged.
All accounts payments can be seen by logging into your parent portal. If you are having trouble viewing your payments on the parent portal please email the office to confirm your contact information is current.
Monthly tuition is pro-rated if registration is completed after the first class of the month. All Registration Fees and/or tuition are NOT refunded or refundable. Tuition will be AUTO DRAFTED on the 1st of each month using the credit card provided at registration. Should a parent/legal guardian wish to use another method for payment, they are welcome to do so but payment MUST BE MADE before the 1st to avoid the auto draft if there is a card on file and MUST BE MADE before the 3rd to avoid accruing late fees. (NO EXCEPTIONS). Tuition and/or registration fees cannot and will not be refunded. Failure to make payment by the 3rd (or update credit cards) will result in a $15 late fee in addition to a $5 daily late fee for each day the balance remains unpaid until the tuition is paid in full.
A $35 fee is assessed for any and all returned checks.

This is a release of liability and waiver of certain legal rights.

As the enrolled participant and/or the parent/legal guardian of the enrolled participant/student/dancer, I agree and understand that dance/fitness training is a potentially hazardous activity. I recognize that there are risks inherent in dance training including but not limited to serious physical injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer/student/participant and parent/legal guardian) assume all risks related to the use of any and all spaces used by MoveNation Dance Academy, LLC. The participant/student/dancer hereby agrees to participate in activities of the MoveNation Dance Academy, LLC and hereby agrees to indemnify and hold harmless MoveNation Dance Academy, its instructors, officers, directors, partners, agents, contractors and employees against any liability resulting from any injury that may occur to the participant/student/dancer while participating in activities of the MoveNation Dance Academy, LLC. The participant also agrees to indemnify MoveNation Dance Academy, LLC for any damages incurred arising from any claims, demand, action or course of action by the participant/student/dancer. I/we will not hold MoveNation Dance Academy, LLC, its instructors, officers, directors, partners, agents, contractors and employees liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by MoveNation Dance Academy, LLC.

The enrolled participant/student/dancer authorizes any representative of MoveNation Dance Academy to have the participant treated in any medical emergency during their participation in activities of the MoveNation Dance Academy, LLC. Further, the participant and/or parent/legal guardian agrees to pay all costs associated with medical care, transportation for the participant/student/dancer.

Photograph/Likeness/Videotape Media Release Waiver

As the enrolled participant/student/dancer and/or the parent/legal guardian of the enrolled participant, I authorize MoveNation Dance Academy, LLC and/or its representative(s), partner(s) agent(s), employee(s), or contractor(s) to photograph and/or videotape and use any photograph/media/likeness of me or my minor child for any purpose, including publicity, choreographic archives, promotional materials and/or any other reason deemed appropriate by MoveNation Dance Academy.

I understand that in order to opt out of having my dancer's photos released publicly, I must notify MoveNation Dance Academy and request a change in status for "No Public Photos" be made to my profile. Once this change is made, your dancer's profile will indicate to MoveNation Dance Academy staff that no public photos should be posted.
