________ ENROLLMENT When enrolling for San Diego Hula Academy’s 2018-2019 program, you are enrolling for the October 2018-September 2019.
________ Because you are holding a very valuable spot in our classes, there are no refunds available for any registration, tuition, or costume fees paid.
________ TUITION Monthly payments are due by the first scheduled class of the month, from October 2018 through September 2019, and a $10 late fee is applied to overdue accounts after the 15th of each month.
________ PHOTO RELEASE: The school is hereby granted permission to take photographs and videos of students to use in brochures, web sites, posters, advertisements and other promotional materials the school creates. Permission is also hereby granted for the school to copyright such photographs in its name.
________ RISK OF DANCE: I understand dance and related activities involve risk of injury. I agree I will not hold San Diego Hula Academy nor Momentum Training Center (dance facility). (its owners, staff and related parties) responsible for injuries/damages incurred by any of my family members while participating or visiting the facility.

REGISTRATION:
San Diego Hula Academy (SDHA) has one registration period for the academic year (October 2018-September 2019). The greatest growth and progress occur when students stay in the same class with the same students for the academic year, just like in school. (Students can always enroll mid-year if there is room in a class.)
A $35 registration fee is due once per year per student.
Registration can be done over the phone, by e-mail or by mail.
Registration at San Diego Hula Academy automatically includes you in the special events, workshops and performances.
TUITION:
Tuition rates are based on the length of the entire academic year, so tuition installments are the same whether there are 1, 2, 3, 4 or 5 classes within the month. Tuition does not change based on attendance. (If registering mid-month, then your first automatic payment will be prorated to reflect your start date.)
Tuition is per month – not per class. The monthly fee for a 1hour class a week is $50.00. Fees are reduced for additional classes per week or for more than one child per family.
Class fees are due at the first scheduled class of each month. A $10 late fee will be applied to accounts not paid by the 15th.
Tuition may be paid by the following methods: cash, money order, check, or credit card (We accept MasterCard, Visa, and Discover).
The parent or guardian should notify San Diego Hula Academy of any change to the credit card or checking account.
The returned check/declined card fee is $35.
All fees paid are nonrefundable and nontransferable.
CANCEL ANYTIME! There’s no long-term contract, so just call 858-243-5071 or stop by 10 days before your billing date to stop future payments.
SWITCH CLASSES WHENEVER YOU LIKE! Interested in a different class next month? Switching classes is easy! Just call 858-243-5071 or come in anytime to ask for a class switch.
MAKE-UP CLASSES Missed your class? Don’t worry! Make-ups only expire at the end of the month. Just drop into any class of your age and level and let the instructor know that this is a make-up class for you. That’s all! I understand that make-up classes are permitted for each class my child misses. Please communicate with the instructors when you plan to miss and will be making up classes.
HÄ€LAU INFORMATION & DRESS CODE:
PUNCTUALITY
Students should be dressed and prepared for class before it starts.
PRACTICE ATTIRE
Hair should always be securely pulled away from the face to not distract the dancers during class time.
For hula, all students should wear a hÄlau shirt and pa’u skirts (for female students) and lawa lawa (for male students) are required. Shorts under pa’u skirts for female students are necessary for modesty.
For Tahitian, students are encouraged to wear form fitting dance attire to display proper body alignment and allow for easy movement. Pareo/sarong are also requested. We will have some pareo on site for those that need to borrow.
No street shoes should be worn in the studios.
No food or drinks of any kind are allowed inside the dance studios.
Chewing gum is not allowed inside the dance studios.
RISK OF DANCE
I agree that I will not hold San Diego Hula Academy or any faculty member, or employee of the same, liable for injuries sustained or illness contracted by me while a student of San Diego Hula Academy.
CHILD SUPERVISION
Parents or guardians are strongly encouraged to remain in the dance studio during their child's dance class(es) for announcements and progress updates. Parents and guardians of dance students 8 years old and younger must remain in the building during their child's class(es).
MEDIA PERMISSION
I hereby grant San Diego Hula Academy and their agents permission to use my child’s likeness in photographs and/or videos in any and all of its publications, including but not limited to, their website, without payment or any other consideration. The likeness may be intact or in part, composite, altered or distorted form without restrictions.
CLASS CANCELLATIONS
In the unlikely event of a class cancellation as a result of adverse weather conditions, dance class cancellations will be posted on our website home page, Facebook, and Instagram. You may also call the school at 858-243-5071 for current information.
ONLINE PRIVACY POLICY AGREEMENT
San Diego Hula Academy (SDHA), is committed to keeping any and all personal information collected of those individuals that visit our website and make use of our online facilities and services accurate, confidential, secure and private. Our privacy policy has been designed and created to ensure those affiliated with San Diego Hula Academy of our commitment and realization of our obligation not only to meet but to exceed most existing privacy standards.
