Enrollment Agreement - Elevate Movement Studio
By enrolling at Elevate Movement Studio for the 2024 Season - Session I (August 19 - December 14, 2024) and / or Session II (January 13 - July 11, 2024), participants agree to the following terms:
1. Payment and Commitment: Tuition is paid monthly or in full to cover instructor fees and operational costs throughout the two sessions per year and annual season. Families commit to enrolling for the entire session or full annual season upon registration.
2. Attendance: Regular attendance is crucial for dance progress. Participants must attend all scheduled classes. Parents must notify the studio in advance of any absences. Excessive absences may result in class placement reevaluation. Students must arrive on time; arriving more than 10 minutes late may result in observing class to avoid disruption. Repeat tardiness may prompt a meeting with instructors and parents.
3. Code of Conduct:Students are expected to show respect toward instructors and peers at all times, including attentive listening, following instructions, and maintaining a positive attitude. Disruptive behavior will not be tolerated and may lead to disciplinary action. Parents are encouraged to foster a positive environment and respect studio policies. Concerns should be addressed privately with studio staff.
4. Health and Safety Guidelines: Students should stay home if feeling unwell, especially with symptoms of contagious illnesses. Please inform the studio of absences due to illness. All injuries must be reported immediately to instructors. An incident report will be completed, and parents will be notified. In case of a serious injury, emergency medical services will be contacted.
5. Dress Code Compliance: Students must adhere to the dress code at all times. Failure to comply may result in sitting out of class until proper attire is worn.
6. Class Make-up Policy: Students may attend make-up classes for missed sessions due to illness, family emergencies, or weather-related closures. To schedule a make-up class, please contact the office. Make-up classes must be scheduled in advance and are subject to availability.
7. Consent for Use of Photos/Videos: Photos and videos of students may be used for marketing purposes. By enrolling, you consent to the use of these images. To opt out, please contact elevatemovementstudio@gmail.com
8. Liability Release: Participation in dance classes carries inherent risks. By enrolling, you release Elevate Movement Studio from liability for injuries or accidents that may occur during classes or events.
9. Withdrawal Policy: To withdraw from a class, please notify the office in writing 30 days in advance. Tuition will be prorated based on the notice period.
10. Communication Policy: Preferential communication is via email. We commit to responding within 24 hours to all inquiries during office hours. Please allow 24-48 hours for a response.
11. Studio Etiquette: Students must demonstrate respect for instructors and peers at all times, maintain a tidy environment, and refrain from bringing food or drinks into the studio, except for water bottles. Changing rooms and designated waiting areas should be used for dressing and resting. Parents and siblings are to wait in designated areas.
12. Lost and Found Policy: Lost items should be reported to the office. Unclaimed items will be held for 30 days before donation. Please check lost and found regularly for any lost items.
13. Arrival and Pick-up: Please arrive 10 minutes before scheduled classes and ensure timely pick-up. In case of delay, please call the studio.
14. Attendance, Absences, and Make-up Classes: Regular attendance is crucial for community and technical development. Notify the studio of absences through the Parent Portal or direct contact. Only four make-up classes per session are allowed, scheduled through the Parent Portal or contacting us within 24 hours. Make-up classes must be at the same level or lower and expire at the end of the session or upon class withdrawal. No refunds or credits are offered for unused make-up classes.
15. Refund Policy: Registration fees are non-refundable except for medical emergencies with a doctor's certificate. Refund requests must be submitted in writing. Costume fees are non-refundable after ordering. Recital fees are refundable up to 30 days before the event. No refunds will be issued after this period. Private lesson fees are refundable with 24-hour cancellation notice. No refunds will be issued for last-minute absences or cancellations.
