Liability Release Form and Assumption of Risk
I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of all spaces used by PROJECT DANCE.
I/we agree to release and hold harmless PROJECT DANCE including its owners, teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold PROJECT DANCE liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes.
Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behaviour in addition to any damage I/we may cause to the facilities utilized by PROJECT DANCE.
I grant permission to the staff of PROJECT DANCE to take first aid or emergency measures as judged necessary for the care and protection of my child while under the supervision of the studio. In case of medical emergency, I understand that my child will be transported to an appropriate medical facility by the local emergency unit for treatment if the emergency unit deems it necessary. I understand that in some medical situations the staff will need to contact the emergency resource before the child’s parent, physician, and or other person acting on the parent’s behalf. I also understand and agree that the child’s parents or legal guardians shall be responsible for any expenses incurred.
Parents/legal guardians give their permission to PROJECT DANCE to use photos and or video of their child without remuneration relating to studio publications, social media, advertising, tv and news coverage.
Tuition Policy
The studio calendar and tuition rate is designed to give a minimum of 30 classes over a ten-month period from September to June. For your convenience, this yearly tuition amount is broken down into ten equal monthly instalments. The instalments are not relative to the number of classes offered in any given month. Due to the calendar and school vacations, some months offer as many as 5 classes, while others may offer 3. All lessons are paid for in advance, due the 1st of each month. You will not receive a bill prior to the due date of tuition. Late notices will be emailed/texted to all past due accounts. There will be a 10% late charge added to any account not paid in full by the 15th of the month. This charge will continue to be added each month that month's tuition payment remains late. There will be no refunds or deductions for lessons missed. We accept cheques (10 post-dated), Visa, MasterCard, Amex or PAD -
Pre-Authorized debit. (NOTE: there is a 3.25% charge on all credit card payments for tuition, competition fees & costumes. There will be a $25 returned check charge for all checks returned from the bank. Any student with an overdue balance of more than 45 days will not be allowed into class until payment in full is received or payment arrangements have been made. ANY CLASS WITHDRAWALS AFTER DECEMBER 31 WILL BE SUBJECT TO THE REMAINING FEES THRU THE END OF THE DANCE YEAR IN JUNE. All accounts with PROJECT DANCE have one primary account holder. This is the account holder that registers and signs the registration form. This account holder will be the one considered financially responsible for the account. Payments will always be applied to outstanding balances first before being applied to current or future charges.
Tuition is non-refundable and non-transferable. There will be no refunds or deductions for classes missed. In the event of a serious medical injury, a request may be made for class tuition to be credited. Approval at the discretion of the Owner/Director. If we are forced to close due to inclement weather, we will try to make up classes. Please understand that we cannot be responsible for acts of Mother Nature or a Pandemic.
Costume Deposits are due: October 15 for Company/Competition ($75 per class) and November 15 for Rec and Mini ($50 per class).
Please note: Costume Deposits are NON-REFUNDABLE.
